Make the Road New York (MRNY) builds the power of immigrant and working-class communities to achieve dignity and justice through organizing, policy innovation, transformative education, and survival services. Make the Road New York operates community centers in Bushwick, Brooklyn; Jackson Heights, Queens; Port Richmond, Staten Island; Brentwood, Long Island and White Plains, Westchester County. With a membership of 24,000, MRNY tackles the critical issues facing our communities, including workplace justice, tenants’ rights, immigrant rights and civil rights, TGNCIQ justice, public education, health care access, and immigration reform. (TGNCIQ = transgender, gender non-conforming, intersex and queer) ). Make the Road New York is in a bargain agreement with National Organization of Legal Services Workers (NOLSW).

MRNY is a multi-service organization. Our member-led organizing committees -- which work on the issues named above -- implement strategies to combat shared problems, and develop leadership and the capacity for civic participation. We also provide an array of high-quality bilingual services: We offer English for Speakers of Other Languages (ESOL), Spanish-language literacy, computer literacy, citizenship preparation, and in-school and after-school youth programs. We have a robust legal program that offers direct representation across a spectrum of practice areas, including employment law, immigration law, housing and benefits, and TGNCIQ civil rights; we also take on impact litigation as a strategy for achieving broad change. Our health program offers facilitated enrollment into health insurance programs and SNAP benefits, a community health worker home visit program, nutrition education and emergency food pantries, health care navigation, and more."

MRNY’s health department promotes the health and well-being of our community members. They advocate for improved access to healthcare for immigrants and provide health services to community members. The health department combines one-on-one assistance, helping individuals and families navigate the health system and apply for health insurance; operates two food pantries, and creates a safe space where TGNCIQ community members can obtain necessary health information. MRNY’s health department also runs a promotora program, training community members to do outreach and screening for food stamps (SNAP) and health insurance benefits and refers eligible families to apply. The health department also runs a community health worker (CHW) training program and has several projects where CHWs do home visits with families who have asthma or other chronic illnesses. The health team also leads campaigns at the city and state level to increase access to care and coverage for immigrants in New York.

Current Opening:

The Community Health Worker (CHW) is based in Staten Island and works full time 40 hours in the Health Department. They conduct home visits to provide asthma education and screen individuals for social determinants of health. The CHW is key in providing care navigation by connecting with and maintaining patients in care by helping people access health care services, embrace positive health behaviors, and navigate complicated treatment regimens for asthma. They maintain case records and tracking documents, conduct weekly outreach, data entry and reporting, and support other health team projects as necessary. Additionally, they connect clients to all MRNY services and events, and the MRNY mission to create collective community power.

Key Responsibilities but not limited to:

• Conduct outreach to potential participants to screen them for care navigation services. Connect participants with appropriate resources and services based on the social determinants of health assessment.  Recruit patients and enroll them into the Asthma Home Visit program. Schedule Home Visits (HVs) with eligible asthma patients. During the Home Visit conduct the following: review the patient's Asthma Action Plan, ACT test, asthma education and reinforcement, environmental assessment.  Maintain contact with patients in the program to ensure they stay in care and revisit patients as necessary.  Assist patients with scheduling doctors appointments and accompanying them to doctors appointment if needed Provide referrals for Integrated Pest Management (IPM), as needed, and referrals to MRNY and other organizations for social services Establish relationships with providers to ensure their knowledge, understanding and referral process to the HV program. Spend time based at clinical sites for warm transfers, as required. Complete case conference with providers to report HV findings, address patient’s needs and collaborate in meetings with MRNY/clinical site.  Conduct data entry and capture notes for all case management in both internal and external databases such as, Salesforce and Channels.  This includes capturing environmental assessments, outreach attempts, patient encounters, asthma severity/control, symptoms/medication, and all notes from findings • during HVs. Complete reports, as needed. Partner with providers, and other organizations participating in this project and attend meetings as required.  Attends all mandatory Staff meetings and other health team department and equipito meetings as required, and maintains required certifications up to date.  Work with other MRNY staff to envision and implement ways to support and mobilize MRNY members around health policy campaigns and org-wide issues. Work with the health team to identify health issues of concern and develop strategy/carry out initiatives to solve systemic problems in the health system Telework, as required.

Requirements:

• Eligible to work in the U.S Required to be fully vaccinated with an FDA approved COVID-19 vaccine prior first day of employment. Fully Bilingual (Spanish/English) MANDATORY; Spoken and Written Completion of Community Health Worker training program required High School Diploma or GED preferred, but not required Knowledge of Microsoft Office & Google Suite  Interpersonal and organizational skills of the highest caliber Relentless drive, tenacity and a willingness to do what it takes to advance our mission and values Critical thinking and problem solving skills Knowledge and familiarity with Staten Island  Flexible work schedule, some evenings or weekends required Ability to work effectively in a fast-paced, high-energy environment with strong personalities, peers, public officials, and allies

Have a working knowledge of spreadsheets, Microsoft Office suite, and using a database (ideally Salesforce) to run reports. Flexible work schedule, some evenings or weekends required Strong verbal and written communication skills High level of accountability and initiative; needs to be a self-starter and eager to do team work to support the organization as a whole

Salary and Benefits:

Starting salary is based on a scale set by a democratically-elected personnel committee and will depend on factors such as experience and education. MRNY offers an excellent benefit package including health insurance, 401K, and generous paid vacation, sick, personal days. The salary range for this position is from $49,023 to $59,023
