GENERAL DESCRIPTION

The primary function of the Outreach Coordinator is to promote the Georgia Transplant Foundation at dialysis clinics, health fairs, and community events; manage the Mentor Project and Trends in Transplant conferences; and create and administer a GTF volunteer program.  The Outreach Coordinator will visit sites to educate patients about the programs and services offered by the Foundation and will develop and maintain relationships with them.  The Coordinator will manage the Mentor Project by training and connecting patients with appropriate mentors.  The Coordinator will plan and facilitate Trends in Transplant conferences throughout the state of Georgia.  The Coordinator will work with the Director of Patient Services and the Executive Director to create a volunteer program to offer GTF clients meaningful involvement with the organization. 

MINIMUM QUALIFICATIONS

  • Bachelor's Degree from an accredited institution and two years of work experience Must have a good understanding of health care issues related to patients and their families or willingness to learn Strong oral and written communication and presentation skills Ability to speak publicly in an articulate, engaging manner Ability to collect and report on data required for grants and other funding Practices confidentiality Must be able to work effectively under deadlines, with multiple tasks, and with individuals representing all levels within the organization Must be skilled at planning, organizing, and prioritizing workload Must have reliable transportation

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Oversee Outreach, The Mentor Project, and TNT Conferences Implement programs created by the Patient Services Director to meet the specific needs of the program Prepare outcomes measurements and track outcomes for projects Provide patient outreach and education at dialysis centers, conferences and transplant hospitals throughout Georgia Foster relationships with and educate dialysis social workers Conduct mentor training workshops Accept mentor referrals and works with lead mentors to assign mentees Plan and facilitate Trends in Transplant conferences

Foster relationships with transplant centers

  • Provide non-medical perspective to educate and support clients with services provided by the Foundation
  • Build relationships in the community through one-on-one networking, group meetings, health fairs and other outreach opportunities to help provide resources for the Foundation’s program participation
  • Ability to work closely with other employees to ensure positive, constructive environment within the Foundation
  • Work with Director of Patient Services to support ongoing foundation programs
  • Perform other duties as assigned

REQUIREMENTS

  • Requires traveling throughout the community; reliable transportation and a valid driver's license are required Appropriate car insurance
