Summary

Partners for HOME is looking to hire a self-motivated go-getter who will manage and implement a variety of projects for the organization. This position is excellent for someone who is flexible; able, and not afraid to take initiative; detail oriented; and looking to contribute to an organization that is at the forefront of making homelessness rare, brief, and nonrecurring in Atlanta.

Essential Job Duties and Functions

The project manager will be responsible for all aspects of work related to the chronically homeless population. Duties are outlined below, and additional duties or responsibilities will be added when needed.

Targeted Population Tracking

  • Maintains By Name Lists (BNL) and serve as subject matter expert on the housing queue and issues related to chronically homeless population
  • Reports out on status of BNL at any given time and uses data from BNL to guide navigation discussion/system needs
  • Ensures relevant housing solutions are identified for each individual on BNLs and that movement toward housing is happening, utilizing resources outside of the Atlanta CE system when necessary
  • Leads system navigation work, identifying system needs and barriers to housing – using that information to guide decision making and navigation agendas
  • Works effectively with HMIS staff to enter data, create reports and other data related tasks
  • Serves on and participates in relevant committees
  • Analyzes trends to provide insight for other staff and agency partners
  • Case conferencing barriers for most acute chronic individuals in SN to find resolution
  • Manage housing queue entries
  • Ensure client information is being entered in HMIS correctly by agency partners

Vacancy monitoring

  • Tracks vacancies, rejections approvals and trends over time
  • Communicate and follow up on matches and referrals with agency case managers and/or assessors to ensure referrals are accepted within 72 hours by CM
  • Communicate and follow up on matches and referrals
  • Create training curriculum and provide trainings for assessors, navigators and providers
  • Monitor client enrollments, vacancy check in and check out
  • Reduce amount of time from referral to project enrollment to move in
  • Monitor provider rejections
  • Make suggestions to policy revision based on trends
  • Monitor PSH and RRH vacancies
  • Ensure timely additions, turnover and resulting referrals
  • Follow up with providers regarding vacancy clean up and maintenance in timely manner

Housing referrals and matching:

  • Maintain project specific knowledge for each RRH and PSH project in Coordinated Entry (for example, documents required for entry, diagnosis requirements, etc)
  • Match clients to vacant RRH and PSH units within a maximum of 72 hours of unit becoming available
  • Develop workflows to refine referral and matching processes
  • Work closely with the HMIS team to handle HMIS tickets as they pertain to CES policy
  • Create and maintain score cards and reports on housing queue and referral process to include statistics

Inventory management

  • Monitor vacancies for RRH including but not limited to: ensuring vacancies referred, enrolled and cleared correctly, all vacancies per project entered in timely manner
  • Manage voucher process with Atlanta Housing and other external entities for all voucher projects coming through Coordinated Entry
  • Reduce amount of time from referral to enrollment
  • Work with providers to manage their vacancy reports and handle any policy related issues
  • Work with providers to maintain accurate vacancies in HMIS – incorporating information from contracts, grant agreements, HIC and GIW
  • Work closely with HMIS to ensure that all new projects are set up completely and accurately, and that all room attributes reflect what is needed for matching.

Other Duties and Functions

As with most positions, while this position will have specific responsibilities, there will be other tasks that arise that the Program Manager will be expected to assist with. This may include:

  • Working with local stakeholders and partners on new or existing initiatives
  • Intersection with other verticals
  • Data entry into HMIS
  • Communicate issues and updates to community partners, government agencies, and other stakeholders
  • Participate as an integral team member of any campaigns or initiatives related to role
  • Create and provide regular and ad hoc list reports
  • Serve as the point of contact for homeless individuals who contact Partners for HOME
  • Answer and follow up on related phone calls and emails
  • Provide information on the CES, including but not limited to accessibility to Coordinated Access Points
  • Refer clients to services available outside of the CES
  • Divert clients to the appropriate CES for their geographic location

Qualifications and Requirements

  • Bachelor degree from an accredited four-year institution
  • One year experience with direct client service (preference given to those with experience in homeless social service)
  • One year experience in data management and reporting
  • Experience working with homeless and housing issues
  • Coordinated entry experience preferred
  • Experience working in nonprofit environment
  • Excellent writing skills
  • Impeccable communication skills
  • Acute attention to detail
