Clinical Director

Reporting to the Board of Directors, the Clinical Director is responsible for the following:

• Oversees all aspects of service delivery and provides direct care management services to clients and their families.
• Provide information to interested parties and conduct consultation and intake sessions with families.
• Develop basic contract and initial plan of care management. Develop long-term care plan as indicated.
• Manage overall care management process and assure high quality of service and reporting for current caseload of approximately 35 PLAN clients.
• Provide direct care management services in the home, community and via phone to a significant portion of the overall caseload.
• Document progress through Simple Practice and billing in Quick-books. 
• Recruit, train and coordinate staff and/or independent contractors to provide additional care management services as needed.
