Human Services Professional (HSP)

Direct Report: Lead Human Services Professional & Program Coordinator 

 

Department: Receiving Center 

 

Hours: 40+ hours per week; work hours will vary to overlap some early mornings, late evenings and weekends; on call rotation required – 1 week per month 

 

Travel Requirements: Moderate 

 

 

Education: Bachelor’s degree in Social Work, Social Sciences, Psychology or any Human Services related field required. Masters degree preferred.

 

Experience: 

 

  • Must have a minimum of two years of prior expericence in Social Services case management.

     

    Must be trauma informed and culturally competent.

     

    Computer proficiency mandatory.

     

    Must be flexible with work schedule.

     

    Experience with reporting in GA SCORE system and SHINES system strongly preferred.

     

    Knowledge of the Department of Children and Family Services (DFCS), Department of Juvenile Justice (DJJ) and Room Board and Watchful Oversight (RBWO) preferred.

     

    Experience providing services to children who are survivors of commercial sexual exploitation (CSEC) or those exposed to trauma preferred.

     

Position Summary: The Human Services Professional (HSP) coordinates services for up to 16 youth at Wellspring Living’s Receiving Hope Center. It is the HSP’s responsibility to ensure that the educational, medical, emotional, and social needs of each participant are met. This position is also responsible for providing and/or coordinating ancillary and social services for participants. The HSP maintains feedback and ongoing communication with participants, guardians, and service providers. The role schedules and may accompany clients to appointments and other obligations. The HSP is responsible for contributing to the program structure and maintaining participant records according to industry standards. 

 

 

Position Duties & Responsibilities (Included but are not limited to):

 

  • Completing and passing the RBWO Foundations 120 hour in class course.

     

    Completing the RBWO Foundations 160 hour new-hire E-training course.

     

    Conducting the intake process for all incoming participants.

     

    Referring youth to medical, dental and other health related services.

     

    Transporting participants to appointments and other obligations if needed.

     

    Providing oversight of medication management.

     

    Assuming on-call duties on a rotating basis to support direct care staff. 

     

    Monitoring and reporting the safety of youth in care.

     

    Fostering a relationship with participants permanent connections, Case workers and other service providers.

     

    Liaison between Wellspring Living and Case Workers.

     

    Advocating for the youth in care.

     

    Conducting the casey life skills assessments.

     

    Developing the Individualized Service Plans for each participant.  

     

    Preparing and submitting accurate and up to date progress monthly summaries for each participant through each of the following documents:

     

    • RBWO Monthly Summary

       

      DJJ Monthly Summary

       

      Independent Living Summary

       

      General Contact 

       

      Every Child Every Month

       

      Every Parent Every Month

       

      Private Monthly Summary

       

Qualifications: 

 

• Participation in all required trainings and competency in areas of training content 

 

• If a master’s level candidate, willingness to pursue or maintain professional licensure by the state of Georgia.

 

• Never have been shown by credible evidence (e.g., a court or jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually exploited, or deprived a child or adult or to have subjected any person to serious injury as a result of intentional or grossly serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application and evidence of having made efforts to obtain and evaluate references from previous employers 

 

• Documentation of at least two professional, educational or personal references that attest to the capability of performing the duties of the position and to the suitability of working with or around children 

 

• Satisfactory preliminary criminal history background check determination and a satisfactory fingerprint records check determination as required by law 

 

• Satisfactory motor vehicle report (MVR) 

 

• Documentation from a licensed physician or other licensed healthcare professional of a health screening examination within thirty (30) days of hiring sufficient in scope to identify conditions that may place residents at risk of infection, injury or improper care 

 

• Current evidence of successful completion of a biennial training program in cardiopulmonary resuscitation (CPR) and a triennial training program in first aid which have been offered by a certified or licensed healthcare professional; such programs shall be completed within the first year of employment
