JOB DESCRIPTION:  Breast and Cervical Cancer Program (BCCP) Coordinator                         

PROGRAM:  Wellness Program

The Center for Black Women’s Wellness is a community-based nonprofit organization that is committed to enhancing the health and well-being of Black women and their families. The organization provides several comprehensive programs that are designed to educate, empower and inspire participants to make positive changes in their lives and the lives of their families.

POSITION OVERVIEW

The Breast and Cervical Cancer Program (BCCP) is an initiative dedicated to providing medically under-served, low-income women with easy access to appropriate breast and cervical cancer screening services and linkages to treatment.  Reporting to the Wellness Program Manager, the BCCP Coordinator is responsible for ensuring the delivery of high quality breast and cervical cancer programming to address the healthcare and health education needs of clients and the community.

ESSENTIAL POSITION RESPONSIBILITIES

  • Coordinate patient scheduling for mobile mammogram services and liaise with service providers to ensure seamless service delivery
  • Oversee mobile mammogram days and provide support to mobile unit and staff
  • Identify, refer and navigate eligible women to breast cancer screening and diagnostic services
  • Facilitate referral process for diagnostic mammograms, ultrasounds, biopsies, or any follow-up treatment as needed
  • Contact patients with abnormal mammograms for follow-up services and provide continuum of care
  • Working with the Nurse Practitioner, connect clients with breast cancer to appropriate medical providers and Georgia Women's Health Medicaid
  • Track patient demographics, mammogram services, referrals, and outcomes on a weekly basis using Mcrosoft Excel and Electronic Medical Records
  • Assess individual patient barriers to cancer screening, diagnostic services, and initiation of cancer treatment
  • Collaboratively implement health systems changes and CDC evidence-based interventions to increase clinic-level breast and cervical cancer screening rates
  • Provide patient education and support
  • Assist with identifying funding resources and contribute to grant proposal writing and report preparation
  • Assist with creating partnerships to strengthen and enhance program services to program participants including developing and leveraging resources
  • Assist with assessment of program objectives and determine best approaches for achieving program objectives in accordance with client/community needs, funder requirements, and agency resources
  • Collaborate with and keep all program staff informed of program activities
  • Perform other duties as assigned and necessary for the successful implementation of the Breast and Cervical Cancer Program and Wellness Program overall

EDUCATION and CERTIFICATION

  • Bachelor’s degree in Public Health, Social Work or related field

QUALIFICAITIONS

  • Minimum of three-years work experience in a service (individual, group or family) environment in women’s health or other related field
  • Program design and database management experience
  • Knowledge of relevant best, promising and evidence-based practices
  • Strong interpersonal skills and high level of emotional intelligence 
  • Excellent verbal and written communication skills
