POSITION TITLE: Family Heart Foundation— Collaborative Learning Network (CLN) Coordinator & Improvement Coach

The Family Heart Foundation has an outstanding opportunity for a Collaborative Learning Network (CLN) Coordinator

 

This fulltime  position can be remote/home based with some travel.

 

POSITION PURPOSE:

The CLN Coordinator will be a key member of the Family Heart Foundation team reporting directly to the Quality/Implementation Specialist.  This position will be primarily responsible for facilitating and leading the execution of the necessary steps to convene the Collaborative Learning Networks hosted by the Family Heart Foundation.  These networks will be multi-stakeholder aligned with Family Heart Foundation’s mission to save generations of families from heart disease through timely identification and improved care of familial hypercholesterolemia (FH) and Lipoprotein(a). The coordinator will work closely with experts and consultants to develop the objectives of each collaborative, the “change” toolkit to be used, the readiness and team criteria for participants, the learning session workshop content, the action period and common metrics to report, weekly check-in/progress reports, webinars for cross-learning, and wrap-up events annually in which the focus is on sustaining the gains achieved and spreading improvement.

 

 

DUTIES AND RESPONSIBILITIES:

  • Oversee all aspects of Collaborative Learning Networks (CNL) that are convened by the Family Heart Foundation Uses the Model for Improvement (MFI) as a roadmap for leading/facilitating Collaborative Learning Networks working to improve FH and LP(a ) diagnosis and management Expands the capabilities of collaborative participants, including team clinicians, managers, administrators, and front-line staff to accomplish specific collaborative objectives by providing them with subject matter knowledge in improvement science and use of data to drive improvement. Engages and facilitates teams in sharing their improvement work and projects with other collaborative participants Coaches teams in the appropriate use of key quality improvement/lean six sigma tools (e.g. Process Mapping, Fishbone, Pareto charts, Run Charts) Coaching teams in identifying high leverage change ideas and testing them using Plan-Do-Study-Act (PDSA) methodology and in implementation of changes Facilitates team planning for completion, control, and sustainability of improvement projects Applies concepts of spread, scale-up, and Shewhart charts
  • Performs miscellaneous job-related duties, as assigned.
  • Complies with the Family Heart Foundation’s Policies and Procedures.

 

 

EDUCATION, TRAINING, CERTIFICATION, LICENSES:

  • Post secondary education in a clinical field (medical, nursing, allied health) or equivalent Evidence of licensure in clinical field in the last five years preferred, active license not required Certification in modern improvement methodology (Lean, Six Sigma, etc.) and/or Professional Project Management and/or Healthcare Quality preferred.

 

 

EXPERIENCE AND SKILLS:

  • Minimum of 5 years clinical experience working in healthcare delivery system Minimum of 5 years of experience leading improvement or system design projects required. Experience leading improvement initiatives in ambulatory clinical care settings is preferred. Prior IHI breakthrough improvement model experience preferred. Excellent verbal and written communication and ability to develop presentations and deliver presentations to moderately sized audiences. Adept at using a collaborative communication style and/or use of “appreciative inquiry” to facilitate individuals and teams learning and reflecting on their work and how to improve it.

 

TECHNICAL COMPETENCY REQUIREMENTS:

  • Microsoft Office (Excel, Word, and PowerPoint), Visio or other flow charting tools Platforms that facilitate remote meetings, networking, and collaboration. Use data for improvement including basic statistical analysis including skill with run charts

 

PHYSICAL REQUIREMENTS AND/OR ENVIRONMENTAL FACTORS

  • Work is remotely performed Travel may be required Extensive Computer Use Occasional night and weekend work schedules

 

COMPENSATION:

  • Midpoint- $90k Range-$70-110k
