Job Details

The key purpose of this position is to assist clients obtain and maintain the best possible housing solution for their individual circumstances. This position assists with housing search and placement, hotel/motel voucher facilitation, manages financial assistance funds (hotel/motel, transportation, etc.), along with leveraging supportive services that will assist the participant household to maintain permanent housing. This position also provides case management services in conjunction with agency and community partners as well as utilizes the HMIS system for appropriate and timely documentation. This position will work with the team to ensure program compliance.

DUTIES AND RESPONSIBILITIES:

· Ensure clients understand their rights and responsibilities as tenants and as program participants Orienting them on all program policies and procedures.

· Provides Case Management for individual clients including, conducting needs assessments, creating individualized service plans, financial counseling & budgeting to achieve self-sufficiency.

· Link client with resources including but not limited to; moving assistance, furniture and furnishings assistance, short term storage, employment, education, substance abuse, mental health services, and other resources and services as needed.

· Work flexibly with clients, landlords and case managers to facilitate linkage of clients with appropriate units of their choice as well as completion of the rental application, rent reasonableness, leasing and housing inspection processes (including habitability and lead based screening) prior to placement of client.

· Negotiate lease terms, utility deposits, security deposits, and move-in fees. Work with landlords to reduce or eliminate as many barriers to entry as possible.

· Serve as a mediator between landlords and clients to quickly resolve any issues of misunderstanding as it pertains to the initial set up of the lease, move in or move out.

· Research relevant housing topics and collect and organize relevant housing information for clients and organizational staff.

· Maintain all client charts and records according to established standards.

· Complete data collection and reporting requirements for various funding sources.

· Participate in staff meetings, in-service training, and workshops deemed appropriate by the RISE Program Manager.

· Ensure that all documentation and recordkeeping is completed in compliance with federal, state and local laws and regulations within allotted timelines.

· Perform other duties as assigned.

SUPERVISORY RESPONSIBILITIES:

· None

QUALIFICATIONS:

  • Bachelor’s degree in Social Work, Human Services or related field and 2+ years of experience working with youth, mental health, substance abuse, disabled individuals, and/or diverse populations; MSW preferred.
  • Proficiency in Microsoft applications including Word, Excel and Outlook, HMIS reporting systems.
