Abode Services, one of the largest and most effective nonprofits working to end homelessness in the Bay Area, is seeking a skilled Senior Housing Services Coordinator.  

 

Job Summary:  

The Senior Housing Services Coordinator provides clinical case management to participants enrolled in the CHSP program aimed at increasing wellness, increasing housing stability, increasing income, and providing referrals for ongoing family support. The CalWORKs Housing Support program (CHSP) provides housing locations, rental subsidy and housing centered case management to families receiving CalWORKs cash aid in Santa Clara County.

How You Make an Impact:  

  • Provide clinical case management services including: 
    • Complete assessments and develop housing stability goals in conjunction with participant and housing service coordinators. 
    • Collaboratively work with participants to develop and work on their housing stability goals.   
    • Provide direct crisis counseling and problem identification.   
    • Assist participants with establishing goals and strategies for increasing self-sufficiency.   
    • Provide referrals to appropriate community agencies and linkage to resources. 
  • Attend administrative, and case conferencing meetings with Abode staff or county partners. 
  • Ensure data is accurate and timely to support program evaluation and outcomes tracking. 
  • Assist the Senior Program Manager, Program Coordinator, and other staff as needed. 

 

How You Meet the Qualifications:  

  • MSW/MS/MA Psychology degree from an accredited university required. 
  • Bilingual in Spanish or other language (Mandarin, Cantonese, Vietnamese, Hindi) preferred. 
  • Must have1 year of field experience working with individuals or families with mental illness, substance use disorder, and/or histories of homelessness/housing instability.   
  • Must have a working knowledge of Mental Health Disorders as well as knowledge and ability to implement the following evidence-based models: housing first principals, Harm Reduction, Strengths Based Case Management and Motivational Interviewing. 
  • Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups, while providing direct services.  
  • Reliable transportation and proof of a valid and current California Driver's License and current insurance along with a clean DMV record required. 
  • Ability to work flexible hours as needed.
