Job Title: Senior Project Manager, Housing Resources
Reports To: Deputy Director of Program Strategy and Implementation
Reporting to and under the direction of the Deputy Director of Program Strategy and Implementation, the Senior Project Manager, Housing Resources liaises with local healthcare providers to train staff and coordinate resources for individuals impacted by homelessness. Develop a coordinated system that connects individuals in the hospital experiencing homelessness to the appropriate and most expedient housing solution.
Roles and Responsibilities:
This position is ideal for a community-building, results-oriented leader who can engage with hospital partners to navigate resources for patients experiencing homelessness. As the Project Manager, you will:
- Provide resources and training to social workers, care coordination team members, and other hospital staff on the process to access housing, bed availability, and support resources for individuals experiencing homeless
- Build and maintain relationships with homeless service partners to ensure support services, emergency shelter bed criteria, and availability are identified and communicated appropriately
- Develop a working knowledge of the HMIS (Homeless Management Information Systems) software ClientTrack to manage vacancy data quality and stay up-to-date on shelter bed availability
- Develop relationships with service providers not utilizing HMIS to understand eligibility criteria, in-take processes, etc.
- Develop and manage the new Housing 101 resource program with components including:
- Development of a printed resource guide
- ARCHI-led training sessions for social workers, community health workers, and other regarding availability of housing resources, housing program updates, supportive services resources, patient needs, etc.
- Meetings that include escalation huddles and partner monitoring
- A recorded orientation session
- Processes for ongoing education and implementation
- Attend community meetings to maintain knowledge of housing and supportive resources
- Participate in partnerships with Atlanta Housing, Partners for HOME, and other agencies, with a special emphasis on the housing first model
- Work with ARCHI in understanding, building, and executing a real-time, rapid referral network with data sharing among clinical providers and community-based social services
- Be an active member of ARCHI’s Community Resource Hub Pilot Program
- Act as a “culture ambassador” working through PfH directives on universally improving the “front door entry” experience at participating shelters and nonprofits
- Perform other duties as assigned
- Comfortable working on-site with homeless and low-income populations
- Experience with community-building and developing, implementing and evaluating systems
- Experience building relationships with a broad range of stakeholders
- Strong organization and project management skills with ability to lead projects to completion
- Ability to excel in an entrepreneurial, fast-paced, diverse, results-oriented environment, and to handle multiple projects simultaneously under tight deadlines
- High level of autonomy with work and ability to meet deadlines with minimal supervision
- Strong communications (verbal/written) and presentation skills to disseminate information to staff; media; and leaders in government, nonprofit, business and community sectors
- Excellent interpersonal skills and ability to serve as representative of PfH within public forums
- Ability to thrive under pressure and consistently meet expectations on tight deadlines, while managing a variety of tasks and relationships
- High energy and a passion for ending homelessness
- High emotional intelligence with the ability to perceive and understand people’s motivations
- Proficiency in Microsoft Word, Excel, PowerPoint, and Teams; SmartSheets; Homeless Management Information System (HMIS)/ClientTrack
Education and Experience Requirements:
- Bachelor’s degree required; Master’s Degree preferred
- A minimum of 3 years’ experience in homeless services or a related field
