Campbell-Stone is seeking to hire one full-time Service Coordinator.  The Service Coordinator coordinates and links residents to needed services that will assist them to age in place, and provides information and makes appropriate referrals to residents and families in all areas of case management for services delivered at Campbell-Stone and in the community at large.

Campbell Stone's not-for-profit and mission-driven approach focuses on quality of life and lifestyle of residents 62 and older. We are committed to supporting each resident's social, psychological, physical and spiritual needs. Qualified residents receive federal housing assistance. The Campbell-Stone entities are 501(c)(3) non-profit corporations sponsored by the Christian Church (Disciples of Christ) in Georgia.

Scope and Purpose:

The Service Coordinator is responsible for helping residents maintain a high quality of life and independence.  The Service Coordinator provides resources that allow residents to more easily obtain the services they need to live well and age in place such as:

  • Assistance with information and referrals
  • Health and wellness screenings
  • Assistance with public benefits and community resources
  • Healthy transitions coordination to/from hospital, rehab & home
  • Educational and personal development programs
  • Development of strong partnerships with health care and service providers or provider agencies
  • Advocacy
  • Emergency/crisis assistance

Applicants should have minimum of a bachelor’s degree in one of the social sciences, education in gerontology and/or experience working with older adults, and excellent case management skills.
