POSITION TITLE: Community Resource and Engagement Care Manager

DEPARTMENT: Center for Youth Wellness

REPORTS TO: Senior Program Manager – Community Resource and Engagement

LOCATION: 3450 3rd Street, Building 2, San Francisco, CA 94124 

ABOUT THE ORGANIZATION

Safe & Sound (formerly the San Francisco Child Abuse Prevention Center) is a children’s advocacy organization working to prevent child abuse and reduce its devastating impact on the child, their family, and our entire community.

We have been a pioneer in the field of child abuse prevention, working within San Francisco for more 40 years. Recently the organization has begun to expand its impact working throughout the state of California.

Child Abuse is a complex problem, but a preventable one. Our data-informed approach leverages three primary strategies: we empower children with knowledge and confidence; we support families providing resources and skills; we activate the community and the changemakers within it to create a thriving social safety net for children and their families.

The organization has an annual operating budget of approximately $8 million and is supported by approximately 60 staff, interns, and more than 80 volunteers. Safe & Sound’s headquarters is located in a historic firehouse in the Haight-Ashbury neighborhood of San Francisco, with a second office in the Bayview neighborhood.

OVERALL SUMMARY
This position is responsible for providing Short-term Case Management (STCM), which may include Information & Referral Services, Service Connection, Case Coordination, and other Supportive Family Services depending on each family’s particular circumstances and needs.  This position is full-time and requires a degree of flexibility in work hours due to the unpredictable schedules of families engaging in STCM services.This position reports to the Senior Program Manager - Community Resource and Engagement.

ESSENTIAL FUNCTIONS

Case Management 

  • Identify parents/caregivers through relationships forged with Bayview Hunters Point Community Based Organizations that either do not offer, or have limited capacity to offer, trauma-informed care management..
  • Administer a Pre/post assessment tool to identify familial stressors, barriers, and strengths.
  • Work with ten (10) families at a time, with a minimum of 30 families served, to provide trauma informed short term care management for up to three months per family.
  • Support families at times convenient to them including evenings and weekends. This may include home visits and appointments out in the community.
  • Work in collaboration with each family to develop Family Action Plans.

Collect data, assess progress, refine goals and routinely follow up with each family.

 

  • Maintain regular contact and communication with the Senior Program Manager- Community Resource and Engagement.
  • Collaborate with Clinical Care Coordinators and service providers from Community Based Organizations to provide care management, information and to make referrals, in keeping with agreed-upon service plans.

Service Connection

  • Provide hands-on support in connecting up to 45 participants to resources they identify as a need.
  • Work closely with participants and referral sources to ensure linkages are made. 
  • Families needing more case management will be referred to other community providers including other departments within Safe & Sound.

 

 

General Program Operations

  • Keeps records of all services provided in compliance with legal, agency, and funding requirements.
  • Participate in outreach activities as well as provider and community meetings to increase referrals and community awareness of the program.
  • Attend all other staff meetings and trainings as directed by the supervisor.
  • Assist and participate in collecting and reporting data needed to track families’ progress toward outcomes.

Undertake other duties as assigned.

ESSENTIAL QUALIFICATIONS

  • At least three (3) or more years of relevant experience preferably in a non-profit social service environment.
  • Bayview Hunters Point resident, or deep roots in the community.
  • Ability to work with cultural humility.
  • Ability to complete orientation of Adverse Childhood Experiences and toxic stress science, Protective Factors and Domains of Wellness.
  • Ability to work independently with little supervision. 
  • Ability to build effective working relationships with staff, set clear expectations, and provide both positive and negative feedback in a timely manner. 
  • Ability to effectively collaborate with clients to complete assessments and develop appropriate action plans.
  • Ability to use both qualitative and quantitative data to inform service planning and monitor client progress towards achieving outcomes/goals. 
  • Ability to work with diverse cultural backgrounds of staff and client populations.
  • Ability to prioritize work effectively in an environment with multiple competing priorities. 
  • Ability to work tactfully and effectively with representatives of city agencies, community-based organizations, and the community at large. 
  • Ability to operate within and lead a complex service delivery structure.
  • Ability to handle crisis situations.
  • Ability to use excellent time management and organizational skills. 
  • Ability to use computer skills: word processing, using digital recordkeeping systems to collect and track data, and internet search capabilities.

PREFERRED QUALIFICATIONS

  • Knowledge of community resources in San Francisco
  • Previous experience working with families that are underserved, marginalized and high risk with trauma histories strongly preferred.

This position is funded through a grant with a projected end date of 6/30/2022 and may be extended through 6/30/2023. The base salary for this position is $54, 893.31 annually and is eligible for agency-sponsored benefits.
