Under the direction of Business Development and in compliance with the organization's policies and procedures,

ensures seamless patient transition into agency’s services by interfacing with hospital/facility staff, physicians, and

families to coordinate and facilitate patient referrals.

Job Duties

  • Instruct staff on their responsibility in following the Compliance and Ethics Program.
  • Detect non-compliance and follow up as appropriate with coaching, education and counseling.
  • Completes annual and as needed supervisory visits and annual evaluations.
  • Acknowledges staff when providing excellent care and receiving customer compliments.
  • Develops, directs, and evaluates high quality, professional services.
  • Seeks out new innovative methods and sources to build the volume and broaden the scope of admissions services.
  • Maintains trending data and reports, analyzes trends and implements actions to improve referral management process.
  • Acts as the in-office clinical leader to represent the agency via telephone with referrals for patient care from physicians, hospitals, facilities, and other sources in the community.
  • Responds to communication regarding potential referrals for services with an extremely customer friendly and professional service orientation at all times.
  • Applies professional expertise to accurately completing referral documentation as necessary to facilitate the admission process.
  • Reviews documentation, including orders, and contacts referral sources for clarification of orders or other documentation to determine clinical eligibility for hospice care.
  • Ensures contact with patient/family as soon as possible after referral is received.
  • Ensures all appropriate pharmacy, medical equipment, and supplies are coordinated.
  • Works closely with leadership and peers to run the day-to-day business development operations.
  • Works closely with Medical Directors and clinical leadership to facilitate discussion about referrals during the referral-to-admission process.
  • Builds strong working relationships with hospitals, nursing facilities, home care agencies, and business and professional organizations to foster the mission of the organization.
  • Incorporates best practice to meet organizational and patient care needs.
  • Ensures that appropriate staffing levels are maintained in all areas of responsibility.
  • Coordinates all survey processes, including State, Federal, Joint Commission and others as needed.
  • Continuously seeks ways to improve the referral, admission processes and delivery of care.
  • Oversees the processing of admissions in HCHB, participating in the process as need arises.
  • Identifies and implements innovations in practice.
  • Provides leadership and support to nurses and referral coordinators.
  • Works in collaboration with medical staff, directors, managers, administrators and clinical staff to form good working relationships, promote and perform best practices and achieve departmental and organizational goals.
  • Assists staff in identifying and prioritizing problems, implementing solutions and evaluates effectiveness.
  • Provides professional coaching and mentoring to the staff.
  • Assesses learning needs of direct reports and provides learning resources to meet those needs.
  • Objectively interprets Hospice policy and guidelines to managers and staff.
  • Delegates authority and gives clear instructions and expectations to staff.
  • Acts in a leadership role and consistently contributes to attainment of department and organizational goals.
  • Develops policies, procedures, processes necessary to meet standards and regulatory guidelines.
  • Identifies methods to effectively manage health care costs.
  • Supports staff in their participation in organizational and professional activities.
  • Collaborates with marketing team to support outreach initiatives targeting professional referral sources.
  • Meets goals and expectations regarding conversion rate and admission budget.
  • Other duties as assigned, including admitting patients, if needed to meet the agency’s commitment to the community.

Computer Skills:

Knowledge of basic typing skills, data entry and word processing software. Database software, EMR software, and

general knowledge of Microsoft Office applications is required.

Supervisory Responsibilities:

Manages clinical liaisons, Patient Access Center staff, and admission nurses staff. Is responsible for the overall

direction, coordination, and evaluation of Clinical Liaisons, Patient Access and Admissions. Carries out supervisory

responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include

interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance;

rewarding and disciplining employees; addressing complaints and resolving problems.

Education/Experience:

Bachelor’s degree from a four-year college with 2-3 years hospice/home care and management experience or

equivalent education/experience combination considered. Master's degree from four-year accredited college or

university with hospice/home care and management experience preferred.

Language Skills:

Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.

Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business

community. Ability to write speeches and articles for publication that conform to prescribed style and format. Ability

to effectively present information to top management, public groups, and/or boards of directors.

Certificates, Licenses, Registrations:

Valid Florida driver’s license; licensed as a registered nurse in the State of Florida. Certification as Hospice and

Palliative Care Nurse required within two years of position.

required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability

to adjust focus.
