Diabetes and Nutrition Education Contractors

BASIC FUNCTION:   Come Grow with Us! The Diabetes Association of Atlanta (DAA) is experiencing phenomenal growth in response to the community striving to live healthier lives! We invite you to join our team as a Contract Diabetes and Nutrition Educator. You will serve with other Educators that provide diabetes and nutrition education for all ages through virtual and socially distant, in person platforms.   

MAJOR RESPONSIBILITIES/DUTIES:

• Carry out the requirements of the American Diabetes Association (ADA) Diabetes Self-Management Education and Support (DSMES) program to maintain DAA’s accreditation
• Present diabetes prevention and management overviews for corporations, community partners, physicians and clinics
• Coordinates and presents diabetes education and nutrition education classes through online and socially distant, in person platforms
• Provides assessment, teaching and follow-up instruction to program participants
• Collects and inputs pre and post health data
• Collaborates with Development Team to implement and carry out grant objectives
• Participates in orientation and continuing education for education program
• Coordinates CQI (continuous quality improvement) with collaboration from other staff
• Manage patient data within DAA platforms such as Healthie, Salesforce and Excel
• Create innovative strategies to improve patient outcomes and follow up response
• Retrieve and reconcile data from patient evaluations and surveys

QUALIFICATIONS:

• CDE preferred
• RN or RD with current GA license/registration
• Minimum BS degree
• Proficiency with social media and online educational tools preferred
• Minimum 3 years background in nutrition education preferred
• Minimum 3 years background in diabetes education preferred
• Minimum one year program management experience
• Strong skills in leadership, communication and education
• Willing to travel within agency’s service area (Atlanta Metropolitan area)
