Description

The Housing Navigator is responsible for the administration of the coordinated entry system and other relevant intake procedures. Reporting to the Norcross Assessment Center Director, this position will be focused on coordinated entry system operations. Additionally, this position will work with partners within the system to provide clients with housing solutions to their homelessness crisis.

Position Summary

The Housing Navigator will work closely with clients to identify housing needs and solutions. The incumbent will administer standardized assessments and determine the appropriate levels and types of support. This position will utilize the Homeless Management Information Systems (HMIS) database to accurately record information and make referrals to coordinated entry partners.

Key Responsibilities

  • Providing information to persons experiencing homeless regarding the services available through the Gwinnett County Coordinated Entry System;
  • Administer assessments using the VI-SPDAT Assessment Tool and other eligibility screening tools to determine housing needs and program eligibility;
  • Conduct intake appointments for people experiencing homelessness for entry into the Gwinnett County Coordinated Entry System homelessness;
  • Aiding individuals by making referrals to appropriate housing and services through the Gwinnett County Coordinated Entry System;
  • Creating and maintaining electronic (HMIS) case files and other documentation;
  • Entering data into HMIS each work day in accordance with HUD’s Data Standards for homeless programs;
  • Identifying creative solutions to divert people from entering the Gwinnett Coordinated Entry System when possible;
  • Obtaining feedback from clients that can be used to improve the Gwinnett County Coordinated Entry System;
  • Protecting the privacy of personal data as required by the Gwinnett County Coordinated Entry System;
  • Identifying and maintaining timely information on available community resources, including housing options appropriate for each individual who seeks assistance through the Gwinnett County Coordinated Entry System;
  • Developing and maintaining relationships with local housing partners and serving as a liaison with such entities;
  • Participating in facility staff meetings, case conferences, training and other system activities;
  • Assisting the Norcross Assessment Center Director in the development and implementation of the Gwinnett County Coordinated Entry System initial policies and operating procedures, and updates for the Norcross Assessment Center, as assigned;
  • Attending/participating in workshops, in-service training, conferences and meeting, as assigned; and
  • Performing all other duties, as assigned.

Qualifications and Expectations

Qualifications

  • Bachelor's Degree in Social Work or a related field of study from an accredited public or private college or university;
  • Minimum two years' related work experience;
  • Intermediate level of proficiency with Microsoft Office Professional 2013 or later version is required (additional proficiency with database applications preferred;
  • Must be a creative problem solver who can adapt to rapid changes; and
  • Must be dedicated to the mission and committed to serving clients respectfully and with compassion

Expectations

A successful individual in this role will have:

  • Intermediate level of language proficiency in conversational Spanish;
  • Demonstrated experience in performing analysis of data and in the preparation of reports describing the results of the data analysis;
  • Demonstrated advanced level of knowledge into the availability of local community resources;
  • Proven organizational skills and keenly developed attention to detail;
  • Strong verbal, interpersonal and written communication skills;
  • Ability to work in a team environment;
  • Experience with homelessness and/or affordable housing issues;
  • Exceptional customer service skills;
  • Experience in performing data entry in a networked environment;
  • Conflict resolution skills;
  • Ability to work comfortably with diverse populations, specifically with persons experiencing homelessness;
  • Ability to be self-motivated and capable of accomplishing tasks and projects with minimal oversight; and
  • Demonstrated ability to multi-task, successfully implement time management skills, and complete assignments and projects as assigned.
