Case Manager for Documentation Job Description

Crossroads Community Ministries, a non-profit provider of supportive services for individuals experiencing homelessness located in Atlanta, GA, is seeking qualified applicants for the position of Case Manager for Documentation (full-time).  Applicants must have, at minimum, a BSW (MSW preferred) and 5 years experience in providing case management (preferably for those experiencing homelessness). Certification in ClientTrack HMIS required.  Applicants must have their own vehicle in good working condition and insured as travel to mobile sites to provide services will be required. Applicants must also be willing to use their personal cell phone in the performance of job duties.

The Case Manager for Documentation will be responsible for facilitating documentation services and partnering with community resources to address the documentation needs of an assigned case load.  The Case Manager for Documentation will report directly to the Director of Operations.  General case management responsibilities include, but are not limited to, client screening, intake, assessment, service plan development, monitoring, linkage to appropriate community resources, follow up, appropriate discharge and tracking. We incorporate five components in case management.

  • Evaluation of current service plan
  • Continued program participation
  • Development of transition plan
  • Upon completion of program case manager will follow up every 30, 60 and 90 days with each client.

Guidelines:

  1. Provide on-going assessment and support.
  2. Maintain thorough and complete record of client’s activity/progress.
  3. Accurately identify and prioritize at risk client population.
  4. Document results of above components in HMIS.
  5. Demonstrate knowledge and skill necessary to provide service appropriate to the age/maturity/mental capacity of the client.
  6. Change practice behavior as a result of data by client.
  7. Prepare and submit reports to supervisor as requested.
  8. Handle difficult situations in a discreet manner.
  9. Set goals and time frames for achievement of goals.  Also set goals for transition to permanent or support housing.

The Case Manager for Documentation will primarily be responsible for assisting clients in securing birth certificates and state issued ID's.  Responsibilities for documentation assistance include, but are not limited to:

  1. Helping individuals experiencing homelessness with obtaining vital records (birth certificate and state identification) via mobile site services and at the CCM site.
  2. Creating and sustaining relationships with partner organizations within the area.
  3. Inputting orientations into ClientTrack after providing services.
  4. Documenting services provided into ClientTrack, documenting when the documents have been received by Crossroads and when they were returned to the guest.
  5. Tracking the status of birth certificates for guest ordering birth certificates from out of state.
  6. Educating guests on what is needed to obtain their state ID prior to them visiting DDS.
  7. Creating a schedule that flows and meets the needs of the clients in our community. This includes meeting guest at CCM site or visiting centers to assist individuals with birth certificate needs.
  8. Taking advantage of various trainings that will educate case managers on how to better communicate and serve those experiencing homelessness.
  9. Collaborating with Crossroads team members to serve our guests and provide a lending hand when needed.
  10. Providing in-office assistance when I am not scheduled to provide mobile documentation services.
  11. Identifying resources (attorneys and other avenues) to apply for birth certificates for individuals experiencing homelessness.
  12. Educating yourself on each state's protocol for applying for birth certificates. This enables Crossroads to apply for more birth certificates on behalf of our guests.
  13. Picking up GA Birth Certificates.
  14. Being kind, empathetic and understanding to those you serve. It is a very tough journey, and the documentation specialist must realize they are not here to just provide a service. Our services sometimes include providing a listening ear or message of hope.
