The Agency is a well-respected nonprofit mental health agency with multiple sites in Alameda County.  The Agency is also a social rehabilitation agency providing a full range of Medi-Cal services to adults with co-occurring serious mental health and substance use disorders.

We provide an intensive interprofessional and integrated approach to client care by incorporating behavioral health, primary care, social work, pharmacy, nutrition, occupational services, housing services, peer support, and wellness interventions to comprehensively meet client needs on their path to recovery.

Position Summary:

The Director of Health Care Services is accountable for the establishment, development, and execution of integrated health care services across The Agency, including medical and behavioral health. The Director oversees all aspects of nurse practitioner clinical care delivery, including strategic planning, resource allocation, growth and development, regulatory compliance, clinical quality, and engagement. The incumbent is a member of the Executive leadership team. The Director works closely with the leadership team to create a culture where multi-disciplinary teams are promoted as a means to achieve excellent client outcomes and effective use of organizational and community resources. The Director liaises with and receives regular clinical supervision from a licensed physician, and shares responsibility for clinically supervising staff with a physician. This position proactively identifies opportunities to improve the client and family experience, quality of care, and the overall effectiveness and efficiency of The Agency’s NP clinical services that meet CARF and Health Resources and Service Administration (HRSA) standards.

Qualifications:

       Education, certification and licensure:

  • Master’s Degree in Nursing from an accredited Nurse Practitioner Program, valid California Registered Nurse license and Nurse Practitioner certificate, valid DEA registration (Schedule II-V), and valid California Furnishing Number.
  • Board certification by ANCC as a Psychiatric Mental Health Nurse Practitioner; alternatively, board certification from ANCC or AANP as an Adult, Adult-Gerontology, or Family Nurse Practitioner with at least two years of experience with individuals with severe and persistent mental illness as a registered nurse or nurse practitioner
  • Active BLS for Healthcare Providers certification
  • Active malpractice insurance (reimbursable up to % time worked for The Agency)
  • Must meet Community Care Licensing (CCL) standards.

        Clinical practice:

  • Five or more years’ experience as a PMHNP.
  • Experience in a community based organization (CBO) setting.
  • Extensive experience with diagnostic assessment, risk assessment, medication management, laboratory monitoring, and psychotherapy.
  • Ability to provide effective consultation related to complex medical and psychiatric conditions.
  • Experience working with underserved populations and maintaining cultural competency with regard to diversity in race, ethnicity, language, age, sex, sexual orientation, socioeconomic status, disability, and other groups defined by federal and state law.

        Leadership and supervisory experience and abilities:

  • Experience with NP staff supervision, including NP staff at satellite sites, and comfort with using Telehealth.
  • Knowledge of federal, state and other applicable standards for NP clinical practice.
  • Strong analytical skills, communication skills, conflict resolution, problem identification and resolution skills, and decision-making ability.
  • Demonstrated ability to use automated systems, including electronic medical records/electronic health records and MS Office products such as Word, Excel, and Outlook. Uses informatics and data analytics to help inform the decision-making process.
  • Familiarity with current ICD, DSM, and CPT coding, including the ability to train NPs on these topics.
  • Ability to travel for training, county and stakeholder meetings, collaboration with other organizations & agencies, oversight of satellite clinics, and to other areas of the community as necessary.
  • Identifies and facilitates staff education and training on topics relevant to staff and consumers’ needs.

        Preferred:

  • 5 years of experience in health care administration at a senior leadership level; experience with community based organizations and managed care preferred.
  • A deep knowledge of integrated behavioral health, primary care, population health management, value-based payment, and CMS standards.
  • Experience supporting clinical teams and implementing performance improvement activities.
  • Demonstrated experience developing and implementing quality improvement programs and population health management.
  • Knowledge of federal and state initiatives impacting behavioral health community and primary care.
  • Familiarity with Assertive Community Treatment (ACT), Intensive Case Management, Wellness Centers, Residential Treatment Center models, and crisis intervention care delivery, and willingness to learn and implement other models.
  • Experience with the development of Work Flows and Standard Work to help incorporate Lean principles.

       Essential Functions/Skills:

       Clinical Practice (approximately 50% of time):

  • Assesses, diagnoses, treats, monitors, and educates individuals with psychiatric and substance use disorders.
  • Provides at least 4 direct service hours of medication management and psychotherapy services per every 8 clinical practice hours.
  • Provides crisis assessment and manages level of risk for consumers in distress with occasional after hour availability.
  • Assists with the completion and/or review of ANSAs, CQRT, and treatment plans.
  • Knowledge and effective use of ICD and DSM diagnoses.
  • Collaborates with primary care and specialty providers for comprehensive whole-person care.
  • Utilizes the Agency’s electronic health record, and adheres to Medicare and Medi-Cal’s documentation standards.

       Administrative Functions (approximately 50% of time):

  • Develops the vision, directs, and implements integrated NP team-based care initiatives.
  • Provides executive leadership and a psychiatric prescriber perspective on the Executive Team.
  • Applies psychiatric and medical expertise to continuous quality improvement, the delivery of cost-effective services to our clients, and population health management.
  • Provides clinical supervision and preceptorship to NP staff and NP trainees.
  • Serves on the Critical Incident Review Committee to help guide policy and procedure changes and facilitates Root Cause Analysis (RCAs) for sentinel events.
  • Directs agency involvement in grants related to clinical training of NPs and other health care providers.
  • Represents agency with medical/health care directors and other health care providers and collaborates with Executive Director in developing and maintaining relationships with other health care and community organizations.
