POSITION: Intake / Health Navigation Coordinator, (Full-time), Waivered or Licensed

SALARY: $38.93 per hour ($80,975 annualized) with excellent benefits. Additionally public transit commute and parking expenses reimbursed through June 30, 2021.

REPORTS TO: Director of Clinical Services

SUBORDINATE: Health Navigator

Licensure supervision hours for LCSW, MFT, LPCC, Ph.D., Psy.D. are available for waivered staff. Waivered: Accruing licensure hours as a Registered AMFT, ASW, APCC with California Board of Behavioral Sciences or as a Registered Psychologist with California Board of Psychology.

This position is with the Outpatient Services within the Supportive Housing Program. This program provides housing, case management, and income advocacy for adult residents in nine Single Resident Occupancy (SRO) Hotels and studio apartments in the Tenderloin and South of Market area and scattered-site shared apartments located throughout San Francisco. This position will coordinate the intake process for housing and supervise the health navigation peer staff. We are a committed Equal Opportunity Employer. QUALIFICATIONS: Must be registered or licensed LCSW, LMFT, LPCC, Licensed Psychologist in the state of CA or registered (or qualified to register) with CA Board of Behavioral Science as ASW, AMFT, APCC or Registered Psychologist with CA Board of Psychology. Licensure hours for LCSW, MFT, LPCC or Psychology are available. Must have experience providing direct services including work with individuals who experience mental health, substance use, and physical health conditions. Applicants must have a working knowledge of psychiatric diagnostic assessments, psychological terminology, and mental health and social services in San Francisco. Effective advocacy, organizational, and communication skills are essential. The ability to work as a team member towards common goals and objectives is required. Applicants must have the ability to document using electronic database according to Medi-Cal guidelines. Experience with Medi-Cal billing and AVATAR database preferred. Ability to travel to meetings off-site required. However, personal vehicle is not required. Must successfully complete education and reference verification. After a conditional offer of hire, a criminal background check must be reviewed.

DUTIES AND RESPONSIBILITIES:

Intake Coordinator:

-Review referral for applicants for Outpatient Services in Supportive Housing.

-Liaise with community providers regarding referrals.

-Conduct psychiatric diagnostic assessment and complete treatment plans of care for applicants for clinical eligibility for outpatient services.

-Liaise with Property Management in offering housing to applicants who pass clinical eligibility assessment.

-Liaise with Supportive Housing on-site staff on orienting new residents.

-Complete Medi-Cal documentation on Assessment and Treatment Plan of Care. Health Navigation Coordinator:

-Provide leadership to develop and implement Pacific Clinics/University of Southern California Certified Health Navigation Program, including recruitment and supervision of Health Navigators, under the supervision of Director of Clinical Services.

-Supervise certified Health Navigators.

-Develop and maintain Health Navigation Service database on residents; compile and analyze data; produce and disseminate accurate and timely reports as directed by the Director of Clinical Services.

-Coordinate with Pacific Clinics Training Institute (PCTI) on staff training of Health Navigation Certification.

-Coordinate with PCTI on continuation of Health Navigation Services at Conard House under the supervision of Director of Clinical Services.

*For clinicians who must accrue licensing hours and for licensed clinicians who are interested in direct clinical services, there are opportunities for assignment of a client load for individual and group therapy at Outpatient Services sites.
