JOB SUMMARY: 

On behalf of the liveSAFE Resources, the Rapid Rehousing and Transitional Housing Case Manager is responsible for providing home-based case management services for families experiencing domestic violence, sexual assault, and homelessness.  The participants are rehoused in apartments throughout Cobb County, GA. The Case Managers primary goal is to assist families in addressing the barriers to maintaining permanent housing using a strengths-based, Housing First approach. Functions performed include housing location, family goal planning, connection to community resources, home visits, rental payment monitoring, landlord engagement, and problem solving with other disciplines and Case Managers.  The Case Manager is responsible for encouraging and advising while motivating program participants to accomplish goals in their Service Plan.

DUTIES AND RESPONSIBILITIES:

  • Provide initial assessments, phone assessments, intake assessments on potential families for the Rapid Rehousing and Transitional Housing Programs.
  • Coordinate with Housing Team to locate housing options for families.
  • Develop and update Service Plans and Action Plans with program participants during case management meetings.
  • Assist program participants with setting and prioritizing short- and long-term goals.
  • Complete initial housing inspection of units upon move-in.
  • Complete home visits, when necessary, to assist families in performing the tasks required to maintain housing.
  • Monitor and document on-time rental payments.
  • Conduct regular assessments of family’s progress during case management meetings, including but not limited to, budgeting, employment, education, housing obstacles, medical needs, children’s needs, counseling services, legal matter, and other barriers.  
  • Provide supplies and referrals to community resources as needed.
  • Maintain records in the CaseWorthy database and in the Homeless Management. Information System (HMIS) database for reporting purposes.
  • Maintain file records and documentation of services for audit purposes.
  • Document all interactions with participants and all services provided to the family.
  • Participate in liveSAFE Resources and Housing Program ongoing training opportunities, keep informed on legislative issues that impact survivors of domestic violence, sexual assault, and homelessness.
  • Participate in liveSAFE Resources and Housing Program events and meetings.
  • Participate in multi-agency coalitions and community initiatives.
  • Perform other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of modern office procedures and methods, including telephone communications, office systems, and record keeping.
  • Knowledge of modern business communication, including style and format of letters, memoranda, and reports.
  • Skill to use a personal computer and various software packages.
  • Ability to establish priorities, work independently, and proceed with objectives without supervision.
  • Ability to handle and resolve recurring problems.
  • Strong verbal communication skills and the ability to communicate with persons of different educational levels.
  • Ability to enter routine data elements into manual or automated system.
  • Knowledge of all applicable laws and legal procedures.

MINIMUM CREDENTIALS AND EXPERIENCE:

Bachelor’s Degree required in Human Services, or Social Work or two years minimum experience in related field.  Knowledge of working with Domestic Violence, Sexual Assault, Stalking victims, and homelessness preferred. Must have valid driver’s license in State of Georgia and driving record must comply with insurance requirements. Employment is contingent upon satisfactory results of background, fingerprint, drug, and motor vehicle record checks.
