Make an impact for homeless families by joining Nicholas House, Inc as its Housing Retention Case Manager for our New Horizons Program.  The Nicholas House New Horizons Program provides permanent supportive housing for chronically homeless families with disability. This is a key program of the Nicholas House mission to help homeless families achieve self-sufficiency by providing housing and services via prevention, shelter, rapid re-housing and permanent supportive housing.

The Housing Retention Case Manager assists chronically homeless families with disability to rapidly access permanent supportive housing provided by Nicholas House and maintain ongoing housing stability by working with families to identify and address needs creating housing issues, providing supportive services through collaborative partners and ensuring landlord/community relations. The Housing Retention Case Manager will provide overall assessment, create family service plans, assist families with progress, ensure full/accurate documentation required, and provide referrals and support as needed to achieve goals toward maintaining their own housing without support.

 

QUALIFICATIONS

* Direct experience helping chronically homeless population make progress and maintain housing (family population preferred)

* Experience or familiarity with housing first model of service and use of harm reduction, motivational interviewing, trauma informed care and other best practice methods for serving chronically homeless population

* At least 3 years experience in direct case management, including documentation per HUD standards and use of Client Track HMIS system

* A strong working knowledge of the Georgia human services delivery system, including access to mental health and substance abuse support.

* Experience working with populations that may involve substance abuse, mental health, HIV/Aids or other disability and coordinating service approaches for the needs via collaborative partners

* Must have valid Georgia drivers license and own transportation for use. Driving history and backgrounds will be checked for all final candidates.

* Excellent working knowledge of technology, including use of Client Track HMIS, Excel and other Word Office software

* Excellent writing, editorial and verbal skills

* Ability to work evenings and week-ends as needed 


DUTIES AND RESPONSIBILITIES

* Work with the Coordinated Entry process for both City of Atlanta and DeKalb County to identify appropriate families for intake into program. Complete all eligibility reviews and documentation.

* Manage a caseload of approximately 20 chronically homeless families (approximately 60 persons including children).  Clients are housed in apartment units leased to Nicholas House located in DeKalb County. Conduct regular in-home meetings, engage parents and children in needed services, identify long-term housing barriers and create family service plans to address issues and monitor progress.

* Ensure safety and security of family and dwelling through ongoing home visits and ensure ongoing lease compliance by tenant

* Coordinate all service referrals for clients, including mental heath and substance abuse support and all financial assistance requests for rent or other needs. Work with human services community at-large in connecting resident families with appropriate resources not provided by the agency as needed.  The Case Manger is responsible fore referring resident to necessary and appropriate community resources.  The Case Manager is also responsible for monitoring and documenting resident participation in referral resources. 

* Fully documented all activities, progress and issues in participant file and in Client Track  in anticipation of monthly internal audits

* Develop an ongoing , working relationship with the apartment leasing agent and manager to ensure continued housing stability for all clients.

* Submit monthly reports of program statistical information and general data to be used for grant monitoring and fundraising purposes as well as monthly rental subsidy request or other financial requests as needed for clients.

* When on or off-duty the Case Manager serves as the first point of contact for support of the client or apartment management for issues that may arise that could jeopardize ongoing housing or lease violations. In any crisis situation, the Case Manager ensures that all information relevant to an incident is appropriately documented and recorded.

* Other duties are to be performed as needed and required.
