Description:

The Atlanta/Fulton County Pre-Arrest Diversion (PAD) Initiative seeks a Care Navigator who feels passionately about redirecting people out of the criminal justice system and connecting them to the services that can support their path to wellness.

The primary goal of the PAD Initiative is to divert people away from the criminal justice system who would be better served by social services and other community-based support. Law enforcement officials who have probable cause to make an arrest and believe that the offense is primarily driven by an unmet mental health need, substance use disorder, and/or extreme poverty can offer a diversion referral to PAD instead of arrest. The hallmark of the program is intensive care coordination that offers client-centered, strength-based case management utilizing a Harm Reduction and Housing First approach.

The PAD Care Navigator provides supportive services, case management and system advocacy to diverted individuals (PAD participants) while linking individuals to housing, behavioral health services, and other resources.

Responsibilities:

  • Assist PAD participants with securing supportive services to meet immediate basic needs;
  • Develop person-centered care plans to link PAD participants to appropriate housing, behavioral health, employment, legal, and other services, and accompany to appointments as needed;
  • Manage an average active caseload of 25 participants using PAD’s core values of Housing First, Harm Reduction, and Social Justice;
  • Provide support for PAD participants within criminal justice system, including advocating for participants in court, conducting jail visits, and accompanying to probation as needed;
  • Work closely with the Outreach & Harm Reduction Team to re-engage participants and conduct street-based case management;
  • Maintain all participant charts and records according to established standards and document case progression within the electronic data management system (HMIS/Client Track);
  • As needed, provide diversion referral services to Atlanta Police Department officers;
  • Work to increase and ensure cultural competency (including racial, cultural, gender identity and sexual orientation) within the community;
  • Maintain confidentiality and respect for PAD participants or potential participants related to all personal information, including but not limited to physical and mental health status, gender identity, drug usage, conviction or arrest history, and immigrant status;
  • Commit to ongoing personal, professional, and team leadership development.  

Qualifications:

  • Bachelor’s degree in Social Work, Sociology, Psychology, or Counseling 
  • 2 years of experience in providing case management services;
  • Valid GA Driver’s License required;
  • Experience working with people who are experiencing homelessness, substance addiction, severe and persistent mental illness, and/or extreme poverty, preferably in the Metro Atlanta area;
  • Experience working with formerly incarcerated people, trans people, and LGBQ people to navigate care systems, a plus;
  • Understanding of and commitment to the harm reduction philosophy and a non-judgmental approach to people’s wellness;
  • Excellent communication and interpersonal skills, including the ability to be flexible, remain calm under pressure, and diplomatic in tense situations;
  • Proficient computer skills utilizing database and office suite software.

  • Conduct ongoing street outreach to find and re-engage participants;
  • Assist participants in obtaining documents necessary for housing placement, and addressing barriers that may prevent successful housing placements;
  • As needed, provide on-call diversion response to law enforcement and community calls during referral hours;
  • At time of intake, assess and address emergent/immediate needs of diverted/referred individuals, including need for shelter, food, and clothing;
  • Transport referred individuals in PAD vehicles according to established safety protocol;
  • Work closely with partner agencies to provide reconnection and warm hand-offs for PAD participants;
  • Increase community awareness of harm reduction and housing first principles by providing education to social service agencies and advocating for PAD participants if/when issues arise;
  • Maintain all participant charts and records according to established standards and document case progression within the electronic data management system (HMIS/Client Track).

Qualifications:

  • 2 years of experience in providing case management or community based mental health services to individuals experiencing homelessness or engaged in survival activities;
  • Bachelor’s degree in Social Work, Sociology, Psychology, or Counseling or equivalent experience;
  • Valid Driver’s License required;
  • People with lived experience of incarceration, recovery, homelessness, and those who identify as Trans or LGBQ strongly encouraged to apply.

PAD provides a salary range of $47,000-$50,000, comprehensive benefits, and a strong vacation policy in a people-centered environment.

PAD provides an immediate alternative to arrest for Atlanta Police Department and MARTA police officers who have detained someone for law violations related to mental health, substance use, or extreme poverty. In 2022, PAD will partner with the City of Atlanta, Fulton County, and Grady Hospital to convert a portion of the city jail into a 24/7 diversion center.
