JOB DESCRIPTION: Nurse Care Manager                          

PROGRAM: Atlanta Healthy Start

The Center for Black Women’s Wellness (CBWW) is a community-based nonprofit organization that is committed to enhancing the health and well-being of Black women and their families. The organization provides several comprehensive programs that are designed to educate, empower and inspire participants to make positive changes in their lives and the lives of their families.

POSITION OVERVIEW

The Atlanta Healthy Start Initiative (AHSI) is a case management program designed to improve maternal and child health outcomes in high-risk communities. The Nurse Case Manager assesses pregnant women, infants and children, identifies their health care needs, and develops and implements plans of care to optimize healthy outcomes. This is a part-time position reporting directly to the Program Manager for AHSI.

POSITION RESPONSIBILITIES

  • Provides one on one education using the Partner for Healthy Babies curriculum.
  • Works collaboratively with the Care Management Coordinator and Family Support Workers to provide monthly case management to enrolled Healthy Start participants
  • Documents all client encounters and maintains accurate medical records.
  • Provides care coordination services and education to AHSI participants and their families with links to community resources and services.
  • Prepares clients and their families for transition to appropriate services after 18 
    months. This position works to promote and protect the health and safety of the residents of Fulton County including the priority zip codes in the city of Atlanta.
  • As time permits, will assist the CBWW Wellness Program and Wellness clinic

EDUCATION:

  • A degree from an accredited nursing school, an active Georgia license to practice as a Registered Nurse AND One year of Public Health Nursing experience

QUALIFICATIONS:

  • RN in nursing and 1-3 years of clinical case management or Mother/Baby experience.
  • CPR certification
  • Basic knowledge of Microsoft Office
  • Familiarity with Atlanta’s metro area and surrounding Fulton County (e.g. East Point and College Park)
  • Knowledge of Metro Atlanta community resources
  • Strong interpersonal communication skills
  • Strong organizational skill
  • Access to dependable transportation for home visits to see clients
