JOB DESCRIPTION – Case Manager (Second Chance for Success Mentoring Program)  

REPORT TO: Executive Director

HOURS: Hours are flexible between the hours 9:30 AM- 8:00 PM Monday through Friday. Some weekend work required.  This position will work 25 hours per week.

POSITIONS SUPERVISED: None 

GENERAL DUTIES:  The Case Manager is responsible for coordinating the overall service delivery of the Second Chance for Success Mentoring Program (“Second Chance”). Second Chance targets juvenile offenders and provides them with intensive case management, job training and placement services, education, counseling, mentoring, and other wrap-around supportive services. This position is also responsible for the office management duties of a small office.

 RESPONSIBILITIES

 The primary responsibilities of the Case Manager are:

  • Coordinates program activities including working in a direct service capacity with mentors and participants.
  • Conducts ongoing case management for participants and coordinates service delivery for all services available in-house or via referral.
  • Develops individualized treatment plans for participants and updates as needed.
  • Assists with developing and solidifying community partnerships to recruit mentors and expand/enhance the services provided.
  • Facilitates recruiting, screening, training, and supervision of mentors.
  • “Checks in” regularly with mentoring pairs and offers ongoing support.
  • Maintains confidential files on participants and assures appropriate documentation is filed accordingly.
  • Assist youth in securing employment and training via established partnerships.
  • Coordinates office management duties for small office.
  • Other duties as assigned.

The minimum qualifications for this position is 2 years of previous human services/social services experience and a bachelor’s in psychology, sociology, social work, education, or other related field. We are seeking a candidate that is looking to make an impact.

 

This is an hourly position and the hourly salary range for this position is $16- $18 per hour.
