Position Title: Health Trust Fellow

SUMMARY:

The Professional Development Fellowship with The Health Trust is designed for early to mid-career public health and public policy professionals. There will be four (4) fellows selected to work in the areas of Chronic Conditions, Food, Housing, and Policy. Each fellowship is a two (2) year experience, with the possibility of an additional one (1) year extension. The fellowship is intended to provide participants with the opportunity to work with various community stakeholders/partners in the areas of Chronic Conditions, Food, Housing, and Policy in the Santa Clara County area. The fellowship will also provide professional development opportunities and training, in particular in the area of health equity, with a focus on underserved communities. The fellowship is a two-year public health and learning opportunity with:

 

• Individualized, tailored support to enhance the fellow’s achievement personal and professional goals.

 

• Personalized learning plans and access to learning resources (workshops, mentoring, and community engagement).

 

• Access to broad professional networks to enhance peer learning and knowledge sharing.

Essential Duties & Responsibilities:

Food Fellow

 

• Serve as subject-matter expert in the areas of food insecurity and nutritious food as a vehicle to improve health.

 

• Create a food map to illustrate key food stakeholders, with programs and services.

 

• Participate in food collaboratives and neighborhood association meetings.

 

• Research best practice or nationally recognized programs that address food insecurity and programs that improve health through food.

 

• Keep abreast of new developments and trends in the areas of food insecurity and nutritious food as a vehicle to improve health.

 

• Provide technical assistance to community partners when working on projects that address food insecurity and those that improve health through food.

 

• Provide quarterly presentations/updates to The Health Trust and interested community stakeholders.

 

• Research evaluation metrics that illustrate how access to nutritious food improves health.

 

• Draft briefs, presentations, or memos that summarize trends in food insecurity work.

 

• Meet with appropriate Health Trust grantees and other community stakeholders to learn and support their work.

 

• Identify local grant making and policy advocacy opportunities related to food insecurity or improving health through food.

 

Chronic Conditions Fellow

 

• Serve as subject-matter expert in the areas of diabetes.

 

• Research nationally recognized diabetes programs targeting youth and/or health disparities.

 

• Create a diabetes map that illustrates key stakeholders, with programs and services.

 

• Establish strong relationships with the County of Santa Clara Public Health Department.

 

• Utilize data in conjunction with community input to determine when and how to deliver programs/services that better prevent and manage diabetes.

 

• Actively participate in diabetes collaboratives and commissions.

 

• Keep abreast of new developments and trends in the areas of diabetes.

 

• Provide technical assistance to community partners when working on projects that address diabetes.

 

• Provide quarterly presentations/updates to The Health Trust and interested community stakeholders.

 

• Draft briefs, presentations, or memos that summarize trends in diabetes.

 

• Meet with appropriate The Health Trust grantees and other community stakeholders to learn and support their work.

 

• Identify local grant making and policy advocacy opportunities related to diabetes.

 

Housing Fellow

 

• Serve as subject-matter expert in the area of rapid rehousing, with the opportunity to expand to affordable and/or supportive housing.

 

• Provide technical assistance to housing community partners and/or collaboratives.

 

• Actively participate in rapid rehousing collaboratives and consortiums, with the opportunity to attend other housing-focused collaboratives/consortiums.

 

• Establish strong relationships with housing developers, service providers, and City/County housing departments.

 

• Research nationally recognized and/or best practice models around housing and health models.

 

• Document and disseminate policies and procedures for providing case management, health outcomes tracking, and other key health related support services focused on rapid rehousing housing, with the opportunity to expand to affordable and supportive housing in the future.

 

• Keep abreast of new developments and trends in the areas of housing and health.

 

• Provide quarterly presentations/updates to The Health Trust and interested community stakeholders.

 

• Draft briefs, presentations, or memos that summarize trends in rapid rehousing, with the opportunity to expand to other important/pressing housing trends.

 

• Meet with appropriate The Health Trust grantees and other community stakeholders to learn and support their work.

 

• Identify local grant making and policy advocacy opportunities related to prioritizing health in housing developments.

 

Policy Fellow

 

• Establish relationships with representatives from elected offices from the City of San Jose and the County of Santa Clara. Brief them about The Health Trust’s policy interests.

 

• Network with local and state health policy advocates to stay abreast of local, and when appropriate, state-level health policy issues.

 

• Draft position papers, letters of support, and briefs on policies around food, chronic conditions, and housing.

 

• Utilize social media platforms to inform policy makers, community partners, and the community-at large on important policy issues.

 

• Support The Health Trust and their partner agencies with advocacy efforts.

 

• Provide quarterly presentations/updates to The Health Trust and interested community stakeholders.

 

• Identify opportunities to connect policy activities and priorities with the Food, Chronic Conditions, and Housing fellows.

 

Minimum Qualifications

 

• Bachelors in related field

 

• 3-5 years of professional public health experience working in Chronic Conditions, Food, Housing, or Policy

 

• Experience drafting reports, memos and other documents for targeted audiences

 

• Experience in the review and synthesis of published and unpublished materials

 

• Understanding of program coordination, management, and evaluation

 

• Ability to analyze, think critically, and understand public policies that are relevant to the public health field

 

• Qualitative and quantitative research and data analysis skills

 

• Strong written and verbal communication skills

 

• Strong initiative and ability to solve problems and undertake self-directed work with minimum supervision.

 

• Strong organizational skills and attention to detail

 

• Local travel required. Must have available transportation and ability to commute to various locations other than the assigned office. Some travel to Oakland may be required.

 

Desired Qualifications

 

• Master’s in Public Health, preferred for the Chronic Conditions, Food, and Housing Fellowships

 

• Master’s in Public Policy for the Policy Fellowship

 

• Knowledge and/or experience with Santa Clara/San Benito County, particularly their underserved communities

 

• Bilingual in Spanish or Vietnamese

• This is a full-time (40 hours per week) position based in San Jose, CA.
