Job Details
The Nurse Case Manager position serves to ensure delivery of comprehensive, high quality and cost effective care for agency program participants. The individual in this position manages allocation of medical resources and collaborates with a team of stakeholders (including, but not limited to: program participant, family, program staff, healthcare providers, support coordinators, and other clinicians) to coordinate services and ensure quality care. The Nurse Case Manager assesses, plans, implements, facilitates, monitors and evaluates medical care options and services for individual UCPGA program participants. Responsibilities include coordinate, manage and supervise the implementation of medical/nursing services for all residential and day programs. Maintain medical records in a concise manner, according to agency policies. Assess, train and indirectly supervise non-licensed personnel as needed regarding medical care of individuals served and ensure they possess the knowledge to perform their duties in regards to health care services and medication administration. Education Requirements: Georgia LPN or RN license. Special Licenses, Registration, Certification or skills: must have a valid driver's license and vehicle insurance, and have the ability to utilize Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, etc) Other Requirements: Pass drug test and background check.
