Under general supervision the case manager provides direct services to homeless families who suffer from domestic and/ or family violence and works to enhance housing, family, and economic stability to prepare survivors to make plans to ensure their and/ or their family’s safety.

Perform intake for the transitional housing program, on-going assessment, and develop individualized case plans, goal oriented service plans to enhance safety, stability and self-sufficiency for families and individuals.

Assist individuals in achieving goals through on-site and home-based support; provide advocacy, information and referral concerning health, housing, public entitlements, family functioning and dynamics, and other system issues or service needs.

Assist participants in the development and use of personal and community resources.

Provide resources to help teach and enhance problem-solving and self-advocacy skills.

Function as a group leader for services of domestic violence or family violence to help them learn about and acknowledge abuse and build skills to take appropriate action.

Provide crisis intervention and supports when needed by the transitional housing residents.

Document services provided to clients according to agency policies and procedures.

Assist in outreach and implementation of supports for transitional housing residents who have transitioned from the housing program.

Provide education and outreach to the community on types and availability of the transitional housing services.

Assists victims in knowing their rights about obtaining Temporary Protective Orders.

Provide education to the victim regarding the court process.

Coordinate services with the Department of Family and Children Services, to include working with case manage as necessary.

Maintains an updated resource list and collaborates with other community agencies and partners to help deliver quality services to the domestic violence victim.

Complete all statistical data as required with submission by designated time frames.

Provides transportation for clients by coordinating transportation and or transporting clients to appointments, helping to relocate and other needed services.

Plan and facilitate group learning and role playing in areas to include interview skills, dress for success, building confidence, coping skills, assertiveness skills and residential group living.

To develop discharge/exit plans for clients upon completion of their goals /and or stay in the transitional housing program.

Meet individually with clients to develop appropriate case plans based on need.  Identify and coordinate community resources for victims based on identified need.

To assess/and reassess client needs during their stay in the transitional housing program to determine progress and or lack of progress towards their developed goals.

Collection and monitoring payment of rent and client compliance to program rules and regulations.

To monitor and assess the conditions of the client assigned apartment unit

Other duties as assigned.

Qualifications/Skills

  • Bachelor level education in social services and or related field or;
  • A minimum of two years experience in the field of social services.
  • Excellent communication skills
  • Ability to remain non-judgmental and to work with diverse populations.
  • Organization and follow through
  • Knowledge and experience in domestic violence preferred.
  • Efficient computer skills
  • Ability to work independently and with little direct supervision.
  • Ability to develop and maintain community partnerships and effective collaborative efforts.
