Trinity Center Member Advocate Case Manager
Two part time positions available (24 hours), one of which with a specialization in SUD
The Member advocate manages a caseload of clients from varied cultural backgrounds. Responsibilities include counseling services: prescreening, intake, assessments, orientation, case management, client education, group facilitation, individual sessions, documentation, referrals, and crisis management.
Primary responsibilities:
· Conduct client needs assessments
· Provide service referrals for appropriate supportive services
- Provide services consistent with our mission of treating clients with respect and dignity
· Assist clients in obtaining public assistance entitlements and/or liaise with agencies, as appropriate
- Develop relationships and collaborate with social service, medical, and mental health providers for services that are helpful to our clients to referrals to health care, education and mental health agencies, as needed and conduct follow-up
- Specialize in one or more programs: housing, employment or social services and substance abuse
· Conducting groups and individual sessions including but not limited to therapeutic sessions, processing groups, educational groups, etc.
· Conduct crisis intervention as needed
- Prepare all required case management documentation
Qualifications:
· BA/BS or MSW and have experience in case management/social services or related experience
· 2 years working with homeless or similar population
· Experience providing counseling in treatment group settings
· Familiarity with services provided by Contra Costa County Health Services, Homeless Program, or similar organization
· Excellent organizational and interpersonal skills
· Strong communication, writing and computer skills
· Experience working with low-income housing and jobseekers
· Teamwork leadership ability
· Ability to multi-task, prioritize and meet deadlines
· Fluency in English/Spanish a plus
SUD qualification:
· Registration with Drug and Alcohol Certification recognized by DHCS
