Job Details

Summary:

The Case Manager is primarily responsible for the development and support of the service plans for individuals receiving services through Inspiritus Thrive! Youth Host Home program. As such they serve as an advocate on behalf of these individuals, hosts providing transitional housing, and community partners. The case manager works collaboratively with the Host Home Recruiter to facilitate matching and intake process for youth entering the program as well as facilitating ongoing communication and case management, service referral, and discharge planning. This program emphasizes youth choice and highlights competence in serving youth 18-24 years of age who identify as LGBTQ+ and youth aging out of foster care. Youth will be matched with a host home in the community to provide a supportive home environment for periods of time ranging from a weekend to up to 9 months while the case manager works collaboratively with the youth to reach their goals so they can transition to a more permanent housing solution.

Responsibilities:

  • Program Development: Co-create program policies and procedures with the Children and Family Services Director based on Host Home Program best practices and Inspiritus policies and procedures. Areas to develop include:
    • Intake process and service timeline for youth entering the program
    • Communication practices and relationships with partner organizations to ensure effective service delivery for youth
    • Development of aftercare/follow up procedures for youth exiting the program
  • Program Implementation: Plan and execute activities to implement the Host Home Program. This includes:
    • Participates in case conferencing, interfaces with coordinated entry system, and participates in collaborative process matching youth with hosts
    • Ongoing case management for youth participating in the program
    • Regular communication and collaborative meetings with youth case management partners, funders, and stakeholders
  • Engage youth in Host Home Program consultation and feedback: meet with youth consultants in development of Host Home Program policies and procedures and attend regular meetings
  • Attend staff meetings
  • Utilize data base systems both internally and externally in accordance with program requirements and best practices
  • Attend conferences, trainings and participate in events at other Inspiritus locations
  • Participate in Host Home Program evaluation
  • Cultivate culture of learning for Host Home Program: plan and attend regular, ongoing trainings related to topics connected to Host Home Program including supporting LGBTQ+ youth experiencing housing instability, positive youth development, trauma-informed care, and host home best practices
  • Works collaboratively with other Inspiritus programs providing host home and foster care services
  • Helps recruit and organize community resources to provide training.
  • Monitors and maintains statistics necessary to measure the effectiveness of various recruitment efforts through the use of dashboards
  • Utilizes dashboards to provide reports to Program Managers
  • Conforms with and abides by all regulations, policies, work procedures and instructions.
  • Complies with the various federal, state or local laws that apply to and regulate the job responsibilities.
  • Participates in training, fundraising events and other Inspiritus activities and initiatives such as performance and quality improvement (PQI).
  • Strives toward excellence in the areas of innovation, performance and extraordinary acts

Non-Essential Responsibilities:

  • Performs other duties as assigned to meet program needs.
  • May be cross trained to provide support to and training to hosts as needed
  • May participate in on-call rotation schedule.

Supervisory Responsibilities: None

Financial Responsibilities:

  • Manages funds and other Inspiritus assets, i.e. supplies and equipment in a manner that is considered productive, in the best interest of Inspiritus, and conforms with approved budgets.
  • Works with Inspiritus finance department to ensure payment of youth stipends

Minimum Qualifications:

  • Bachelors degree in human service such as social work or a related field such as sociology, psychology etc. and five (5) years of relevant, recent work experience in working with youth or young adults experiencing homelessness, or an equivalent combination of education and experience.
  • Must possess excellent presentation and training skills
  • Demonstrated experience with working with LGBTQ+ youth of color and commitment to integrating youth voice and best practices working with LGBTQ+ youth of color into all components of Host Home Program
  • Ability to forge a mutually respectful partnership with persons served and their families to help them gain the skills and confidence to address any issues and problems they face.
  • Conviction about the capacity of people to grow and change.
  • Ability to work independently and in partnership with other team members.
  • Must pass a comprehensive background check including work references and criminal background check
  • Must be computer and internet literate with competencies in Microsoft Office Suite (Excel, Word, Outlook) and database/electronic case management system
  • Must have the ability to communicate with all persons in a culturally competent manner.
  • Must be sensitive to the service populations cultural and socioeconomic characteristics.
  • Must hold a valid Georgia Drivers license, possess a reliable personal, multi-passenger vehicle, and have personal auto insurance and a good driving record.
  • Or an equivalent combination of education, experience, competencies, skills and knowledge that ensures the successful completion of the essential job responsibilities.

Desired Qualifications:

  • Masters Degree with 5+ years experience in field of social services, housing, community organizing, or other relevant work; candidates with relevant experience in place of an advanced degree encouraged to apply
  • Project management and/or program development experience
  • Experience working in a collaborative setting
  • Experience recruiting, training, and supporting staff, volunteers, teams, community coalitions, and/or another group working toward a common goal
  • Personal qualities of integrity, credibility, creativity, and a commitment to and passion for organizations mission and the mission of ending youth homelessness

Physical Demands:

Regularly required to talk and hear. Required to stand, walk, sit, use hand to finger, handle or feel objects, tools or controls: and reach with hands and arms. Required to operate personal vehicle.

Employee frequently lifts and or moves 20+ pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.

Working Conditions:

While performing the responsibilities of this job, the employee travels by automobile and is exposed to changing weather conditions. This position may require travel up to 25% of the time locally and throughout the Metro region.
