Job Details
The Child and Family Clinician facilitates Child and Family Planning Meetings using wraparound principals that are family-centered, individualized, strengths-based, and outcome-focused. Santa Clara County Connection’s goal is to help children and their families with complex and enduring needs receive intensive treatment in the least restrictive environment that is appropriate. The Clinician provides assessment of the client and family’s needs and leads the team in providing treatment to support the family toward meeting their goals.

Responsibilities
- Facilitation of Child and Family Planning Meetings. This includes: prepare for the meeting, coordinating attendance, facilitate the meeting, write meeting notes and distribute them within two days, write the mental health notes, and clean up after the meeting
- Case management and coordination of care
- Collaborate with county and community partners including but not limited to county social workers, probation officers, outside therapists, psychiatrists, teachers, etc.
- Complete comprehensive assessments, treatment plan of care, mental health notes, Family Team Meeting notes, Wrap Action Plan, incident reports, and other documentation as necessary and in a thorough, accurate and timely manner
- Intervene with clients’ behavior management programs and provide crisis management as needed. In rare circumstances when there is serious threat of harm, this may include physical management of the clients (implementation of Seneca's Ahimsa Model Training)
- Maintain accurate documentation and paperwork
- Attend team meetings, weekly trainings, individual and group supervisions and adhere to clinical best practices
- Participate in providing emergency on call response as scheduled (typically 24 hours at a time, three times per month)
Qualifications
- Bilingual Spanish/English or Vietnamese/English abilities a plus!
- Master’s degree in psychology, counseling, or social work required.
- Eligible to register with the BBS.
- Own vehicle, California driver’s license and insurance.
- TB test clearance, fingerprinting clearance, and any other licensing or certification requirements.
- Experience in working with children in out of home care preferred
Schedule
- Monday- Friday
- Flexible schedule required, with the ability to work some evening and weekend hours as needed
- Participate in On-Call Crisis Response 24 hour shifts on a rotating basis
