The Medical Case Manager collaborates with an interdisciplinary health care team and other service providers working in the Homeless Health Outreach and Mobile Engagement (HHOME) and Getting to Zero, Intensive Case Management programs. This position offers an opportunity to People living with HIV/AIDS in the City and County of San Francisco to engage in mobile medical and psychosocial services. 

The position introduces and reconnects clients to services in the field and supports them as they begin to engage in a robust mobile program. Clients enrolled in both the HHOME and GTZ intensive case management programs are: individuals experiencing homelessness or who are marginally housed who may not be participating in services; multiply diagnosed with a high behavioral acuity; actively using substances with no intention of decreasing use; refugees, political asylees, immigrants or undocumented individuals; recently released from incarceration; and, actively engaged in palliative care. 

Essential Functions
• Collaborate intensely with interdisciplinary health care team; 
• Conduct field based intake assessment, evaluating the social, emotional, and physical needs of clients;
• Provide case management for higher acuity clients in the HHOME and GTZ intensive case management programs;
• Provide service referral, linkage, coordination and advocacy services for clients, and 
• Educate and support clients to be more familiar with and better access different systems of care (medical, complementary therapy, mental health, substance use, etc.); 
• Participate in the transition of clients to higher and lower levels of care based on acuity; 
• Develop and renew clients' individualized client treatment plans regularly, and
• Coordinate with clients' medical providers weekly; 
• Reinforce treatment goals that clients develop with medical providers and 
• Tailor health education to the client and provide support for antiretroviral initiation and consistent adherence;
• Complete necessary documentation, UOS reports and treatment plans in a timely manner; 
• Participate in weekly case conferences, monthly staff meetings and in-service trainings;
• Perform other duties and tasks as assigned. 

Minimum Qualifications:
• Ability to sit, stand, and walk frequently (this position requires that being in the field at least 50% of the time
• BA in a health sciences or social work-related field or comparable work experience
• Two years of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities. 
• Knowledge of HIV/AIDS, STIs, substance use, mental health, social service and probation system
• Familiarity with HIV/AIDS issues in communities of color.
• Knowledge of harm reduction philosophy, client centered counseling and motivational interviewing techniques
• Must be adaptive, flexible and a team player
• Strong verbal and written proficiency in English 
• Spanish language skills preferred
