Position Title:  Family Services Coordinator

Reports To: Placement Supervisor

Primary Function:

The Family Services Coordinator provides home/parent oversight and support focusing on the ongoing quality, compliance, and efficacy of the programming services.

Provide Placement Supervision

  • Oversee home/parent ensuring post-licensure home regulatory compliance
  • Communicate parent/home regulatory compliance issues with supervisor and Recruitment and Training Staff so accurate evaluations may be completed yearly
  • Communicate family training and support needs to supervisor
  • Communicate home study addendum needs to supervisor
  • Support, coach and encourage family compliance with agency policy and procedures, regulatory entities and best practice standards for child care and intervention 
  • Provide placement management supervision with families utilizing supportive and customer service-focused interventions

Provide Advocacy, Referral and Support Services for Children Placed

  • Participate in Intake Placement Team Meetings at the direction of supervisor
  • Participate in inter-disciplinary team meetings to ensure clear communication about each family and child’s needs and to ensure appropriate coordination of services
  • Advocate for appropriate community supports and services for each child to include (not but limited to) educational, family support, respite, behavioral health, child’s support team and CASA services
  • Communicate with referral source, families and service providers to ensure open and clear communication about family’s and child’s needs
  • Develop and communicate appropriate recommendations for services, interventions and permanency that is based on best practice with the primary focus on the best interest of each child
  • Assist with the coordination of respite ensuring timely communication with families and referral sources while focusing on quality care

Complete and Submit Quality Documentation

  • Complete all documentation required for children in care within required timeframes
  • Complete documentation ensuring documents uphold policy and procedures, regulatory entities and best practice standards for child care and intervention
  • Develop obtainable and relevant goals within each child’s Treatment Plan and monitor goal progress with parent and inter-disciplinary team
  • Document progress toward goals within Case Notes
  • Complete Peer Reviews within required timeframes

Administrative Responsibilities

  • Participate and receive input in supervision
  • Communicate barriers and successes clearly in supervision
  • Complete and submit required outputs and outcome reports within required timeframes
  • Ensure the completion and maintenance of Placement Services Family Services Coordinator documentation
  • Work effectively with state, regulatory, accreditation and funding representatives – ensuring compliance and positive working relations
  • Ensure concerns of families are addressed in a timely and professional manner
  • Ensure services are provided in an ethical and culturally responsive manner
  • Ensure all interaction utilizes customer service principles and practices while conducting agency business
  • Perform other duties as assigned by supervisor for programmatic or administrative purposes
  • Serve as a member of the Placement Services Team supporting agency goals and direction

Qualifications / Education / Experience

An individual must be able to perform each essential duty satisfactorily.  Individual must have reliable transportation; have current vehicle insurance and driver’s license. 

The requirements listed below are representative of the knowledge, skill, and/or ability required:

FSC I: 

Bachelor’s Degree in Social Work (or related field) and at least 4 years of related experience in child welfare and case management or a Masters Degree in Social Work (or related field);

Knowledge of Child Welfare issues; and

Excellent verbal and written communication skills.

FSC II:

Masters Degree in Social Work (or related field) – licensure preferred;

Knowledge of Child Welfare issues;

At least 2 years of related experience in child welfare with case management; and

Excellent verbal and written communication skills.
