PRIMARY DUTIES AND RESPONSIBILITIES:

Housing Navigator and Tenancy and Housing Stabilization (HNTHS) Specialists serve as the homeless household’s advocate throughout the process of locating and obtaining housing and then supporting the tenant to transition to and stabilize in the housing. As a result of ECS continuing to collaborate with the Department of Homelessness and Supportive Housing and other external stakeholders in launching this project, this job description may change both prior to hire, and after, as we continue to implement the best approach and services.  

Specific responsibilities include:

  1. Maintain relationship with clients eligible for housing placement through the placement process
  2. Assist clients in completing housing applications, including obtaining all documents necessary for a successful application, in interviews and in grievance processes should applications be denied.
  3. Support clients in moving into new housing should applications be accepted.
  4. Provide clinical and supportive services to tenants in order to provide them with the support needed to promote stabilization and successful tenancy.
  5. Provide support and education related to the psychological and practical transition of tenants into housing.
  6. Address the change of ‘home’ and familiar location with tenants; the potential for isolation, how to pay bills, manager visitors and relationships, etc.
  7. Arrange for necessary services, such as In-Home Support Services, prescribing primary care provider and/or psychiatrist.
  8. Support tenants in household management skills.
  9. Maintain at least weekly contact for the first month and a at least monthly contact on-going to support linkages to on-site services (if available) and provision of/linkages to community resources, case management and crisis intervention within a housing first and trauma-informed/harm reduction model;
  10. Educate on tenancy requirements and provide support to address barriers to housing retention; and
  11. Identify tenant behaviors that may jeopardize their housing and intervene when necessary to accomplish successful tenancy.
  12. Coach tenants on relationships with landlords and/or other tenants.
  13. Assist with the resolution of disputes.
  14. Advocate and link tenants to eviction-prevention resources around the community.
  15. Interact with third party providers and site-based Tenant Services and Property Management services for shared clients including:

a.                           Timely coordination regarding incident reports and issues impacting housing retention (e.g., late rent payment, habitability issues, refusing pest control) and participation in case conferences and/or housing retention meetings.

 

b.               Participation in site-based coordination meetings;

c.                           Clinical consultation for on-site Support Services and Property Management as needed regarding strategies to address client behavior negatively impacting housing retention.

 

16.   Assist tenants with completing any required processes for housing provider’s certification/re-certification processes.

17.   Update housing support and crisis response plans.

18.   Keep accurate records and charts, enter required data in ONE System

19.   Provide professional, accessible and culturally relevant support, advocacy, information and referrals to program participants

20.   Provide referral services as needed

21.   Work within and promote the integrated services team model in an environment dedicated to harm reduction, recovery, and wellness.

REQUIREMENTS:

  1. Master's degree in counseling psychology (MFT track) or social work (MSW track), with 2 years recent, full time paid experience in assessment, crisis intervention and case management of persons with mental, emotional and behavioral disorders (preferably with the homeless population) preferred.  Candidates BA in psychology, social work or related field, plus 3-5 years full-time paid experience closely related to job description considered.
  2. Substantial or demonstrated knowledge/experience working with individuals in crisis or unstable housing situations.
  3. A strong knowledge of issues confronted by and sensitivity to adults with substance abuse, domestic violence, HIV/AIDS, mental health issues, history of trauma and co-occurring-diagnoses. 
  4. Demonstrated respect for, experience with and willingness to work with a diverse client population. 
  5. Deep knowledge and understanding of the resources available to homeless and low-income residents of San Francisco.
  6. Articulate; exceptional verbal and written communication.
  7. Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in cross-cultural situations.
  8. Professional and discreet demeanor with good interpersonal skills. Ability to handle confidential information and use of tact, respect, discretion and judgement.
  9. Organize material, information, and/or people in a systematic way to optimize efficiency and minimize harm or duplication of efforts.
  10. Experience working in integrated service teams using participant-centered, trauma-informed, harm-reduction approaches. 
  11. Collaborative work style; demonstrated ability to work independently and effectively as an individual and part of a team for successful outcomes.
  12. Excellent Computer skills and knowledge of Microsoft Office software is required.
  13. Knowledge of current permanent supportive housing eligibility requirements, including HUD homeless regulations a plus.   
  14. Knowledge of the ONE System preferred.

OTHER:  Must secure finger image screening and annual TB screening. Must be able to meet physical requirements of the position which may include long periods at a desk and/or computer workstation, daily travel to various sites, and lifting of up to 25 lbs.
