Position:           Case Manager/Family Coach

Position Description

The case manager is responsible for assessing eligible clients for the program, providing case management to residents of Solomon’s Temple with an ultimate goal of leading families to stable housing and self-sufficiency, and to provide effective programming and outcomes.  The Family Coach will conduct initial interviews and intake, demonstrate exemplary resident relations and strive to make families feel welcome, respected and comfortable in their community.  This person will be responsible for general oversight of daily/evening/weekend activities, general office administration and execution of all community rules and regulations.  

Functions

  • This position will provide intensive case management to assigned residents
  • Work as a team with Director of Programs & Facilities
  • Provide instruction and mentoring to families
  • Coordinate referrals & resources to clients on job readiness and employment   
  • Conduct individual skills assessment and barriers to employment
  • Coordinate and/or facilitate individual and group counseling activities to address quality of life issues
  • Network with other agencies to make proper referrals for services such as mental health care, required vital records, financial assistance, legal advocacy, etc.
  • Compile and prepare appropriate program reports
  • Take an proactive approach toward assisting clients with accomplishing goals
  • Ensure proper program compliance
  • Enter data on a timely basis in Client Track
  • Schedule follow up meetings with residents to determine progress
  • Conduct exit exams and host home visits to meet with former residents to determine status and make necessary referrals and recommendations
  • Participate in weekly debrief sessions, monthly supervision, agency team meeting and all required training  development opportunities as determined appropriate by the Program Director
  • Be proactive and work along with schools and make school visits to advocate for children and act as liaison between parents and schools
  • Create relationship close working relationships with families that will transition into a natural after-care process once family transitions to stable housing
  • Conduct exit interviews

Qualifications

  • MSW preferred and/or BSW or related field  
  • Knowledge of case management principles
  • Case management experience serving individuals with experiencing homelessness, mental illness, substance abuse and familiarity with housing related issues
  • Strong interpersonal skills and knowledge of behavior modification techniques
  • Ability to establish and maintain confidentiality
  • Flexibility, strong approach to teamwork
  • Ability to work successfully with minimal supervision 
  • Proof of a valid and current driver license, current auto insurance, acceptable motor vehicle report and a dependable vehicle
  • Willingness to do home visits
  • Proof of citizenship – copy of social security card
  • Intermediate level proficiency of MS word, Excel
  • Knowledge of client track system strongly preferred

Highly organized, self-starter, able to motivate people living in diverse conditions

Articulate, creative person with a proven track record in coordinating multiple tasks

Desire to be very flexible to work with growing non-profit agency

Adhere to all client confidentiality requirements and standards
