The HIV Homeless Outreach and Mobile Engagement (HHOME) program serves the highest need population of people living with HIV who are also severely impacted by homelessness. The multidisciplinary team (medical doctor, nurse, housing case manager, social worker, and peer navigator) provides the delivery quality medical care and social services using a mobile approach. This program is a collaboration between API Wellness, the Homeless Outreach Team, and the Transitions Division of Tom Waddell Urban Health Center (SF Department of Public Health).

The Getting to Zero Intensive Case Management (ICM) program serves high need people living with HIV by providing an intensive level case management, coupled with peer navigation and mobile nursing care. The program works out of the office, but team members spend much of the time mobile in the community. The program is a collaboration between API Wellness and Shanti.

Often clients use the HHOME to stabilize and later transfer to the ICM program.

Position Description: 
The HHOME-ICM Program Manager will provide day-to-day oversight, supervision, and coordination over the all both programs. The Program Manager will also be responsible for directly supervising the HHOME Senior Case Manager (who in turn supervises the HHOME Peer Navigator), the ICM Case Manager, and the ICM Nurse (LVN). The HHOME-ICM Program Manager will report to the Associate Director, overseeing HIV treatment programs.

Essential Functions:
• Serves as day-to-day contact person for all program staff
• Reviews program objectives and performance and designs strategies, as needed, to improve service delivery
• Ensures program deliverables are being met
• Develops and maintains protocols to ensure client engagement in care
• Develops protocols that ensure seamless integration between both programs
• Works with collaborating agencies to ensure strong partnerships are maintained
• Develops, maintains, and supports schedule of API staff, including (1) requests for home visits/outreach needs and (2) open access programming
• Attends and facilitates case conferences weekly for both programs
• Supervises, evaluates and hires direct reports
• Assists with other administrative duties as needed

Minimum Qualifications:
• Bachelor's degree in health or related field. A combination of experience and education will be considered.
• Demonstrated experience supervising and leading teams
• Demonstrated experience facilitating community groups/meetings
• At least 1 year experience with advocacy, counseling, referrals or outreach services in the HIV field
• Familiarity with issues faced by the transgender community, sex workers, and substance users
• Knowledge of HIV transmission and prevention. Knowledge of health and social issues related to HIV transmission in high-risk populations.
• Ability to work with and maintain boundaries with clients and confidential information
• Strong written and verbal communication skills
• Strong organization skills and ability to apply new knowledge into day-to-day client work
• Proficiency in Microsoft Office Programs
• Self-directed, motivated and flexible with the ability to work independently and as part of a team

Preferred Qualifications:
• Bilingual and bicultural 
• 1 year of direct supervision experience 
• 1 year of experience with program design
• 1 year of experience providing intensive case management
• Professional experience working with people living with HIV and those dealing with substance use and mental health complexities
