I. PURPOSE OF THE JOB

The Community Support Coordinator is responsible for assisting in recruiting, training, scheduling, supervising, and conducting performance evaluations of assigned staff.  In addition, the Coordinator engages in resource development for the individuals served and ensures their life goals are implemented, personal outcomes are achieved, and the quality of care is high in every respect.  The Coordinator assists in problem solving, communicating regularly with the people we serve (PS), direct support professionals (DSP’s), families and the Program Director.

II. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Develop detailed knowledge of the needs, preferences, and desired outcomes of assigned individuals through a personal relationship and using tools such as Futures Planning, ISP goals, Personal Outcome Measures. Develop rapport, communicate regularly and identify needs with individuals supported and their DSP”s at least weekly and as needed.
  • Introduce DSP candidates to individuals and families for input into hiring decisions.  Recruit, interview, hire, schedule, train and supervise DSPs for success.  Collaborate with HR on proper hiring and termination practices.
  • Facilitate groups and one-on-one coaching to address initial and ongoing training needs of DSPs.  Determine and document competency of assigned staff.
  • Schedule and facilitate regular household meetings with staff and individuals, assisting in problem solving as needed and celebrating accomplishments.  Ensure staff is focused and well trained on ISP goals and personal outcome measures.
  • Develop and use management strategies that facilitate Person Centered techniques, leadership, teamwork, good will, and problem solving among household staff and individuals.  Proven strategies as a coordinator that work for us are one on one coaching with DSPs and developing a team leader.
  • Communicate regularly with the Coordination Team about individuals and staff issues.
  • Provide supervision to DSPs regularly. Conduct annual professional development and evaluations of DSPs.
  • Manage and keep current on medical, dental and all other appointments of assigned individuals.  Conduct follow-up in a timely manner and ensure all documentation is correct and in their home and office folders.
  • Coordinate activities with external agencies; Support Coordinators, DFCS, Social Security, Advantage, doctor’s offices, community businesses and others, representing the individual you serve and Georgia Options in a professional manner at all times.
  • Develop a resource pool of emergency staff and natural supports who know the individual.  Update the pool as needed to ensure reliability.
  • Respond to crisis situations in which an individual needs emergency care and/or back-up support.  Find and organize staff to fill immediate needs.
  • Provide emergency staffing.
  • Attend meetings and training events to continuously upgrade skills and knowledge of state-of-the-art concepts, particularly personal outcome measures.
  • Work with BCBA and doctors to ensure all behavior support needs are met and staff are trained on and follow the behavior support plan for any PS that requires one.  Assist in developing circles of support for assigned individuals. Assist with scheduling and reviewing staff schedule on Clear Care.  Work with Finance office as needed to ensure payroll accuracy.
  • Manage all required documentation, including evidence of regular supervisory visits. 
  • Do other tasks as needed and/or as assigned such as assist with moving people, help new people settle into their environment.

III. ADDITIONAL EXPECTATIONS

  • Work hours are expected to be flexible and determined firstly by emergency situations and secondly by all other job duties.
  • During non-work hours, the Coordinator will carry a cell phone at all times (unless other arrangements have been made) and will respond to all calls.  The Coordinator makes the judgment whether issues can be resolved by phone.  If emergency home care is needed, the Coordinator first uses the resource list to find back-up coverage.  If coverage cannot be found, the Coordinator provides emergency staffing as needed until other arrangements can be made.
  • The Coordinator will occasionally be relieved of on-call responsibilities during weekend and vacation hours.  These occasions will be negotiated with the Program Director or Executive Director, with a back-up plan in place.
  • The coordinator will occasionally be required to lift, bend, squat, or carry items during his/her shift.
